The licensing coordinator's review of 10 children's records revealed there was no emergency contact listed on the Child Care Registration and Emergency Information form for one child. Corrective action: Child in question now has an emergency contact listed. This was rectified day of the visit. To prevent this in the future, Site Supervisors and Family Advocates will work in tandem, to ensure that at least one emergency contact form is listed prior to a child starting in our program.

The licensing coordinator's review of the daily attendance revealed that there were two staff present and only one staff signed into Classroom 2 and two staff present but no staff signed into Classroom 4. Corrective action: Site Supervisor addressed the sign in and out policy with the staff and sent an email to remind staff to sign in and out on a regular basis. The site supervisor will go over this policy at the next staff meeting on January 29th, 2024.
The licensing coordinator's review of Emergency Operations Plan drills revealed that the program did not practice evacuation off-site and bomb threats in 2023 with all child care staff. Corrective action: A bomb threat drill along with an off-site evacuation will be conducted on Jan 29th, 2024. The center will practice monthly fire drills as well as practice EOP drills. An off-site evacuation and bomb threat drill will be practiced at least once yearly.
The licensing coordinator observed a tote bag accessible to the children in the cubbies in Classroom 4, which contained a nebulizer and Albuterol for an enrolled child. The teacher immediately moved the backpack out of the reach of children. Corrective action: The Health Coordinator emailed staff with a reminder of where medication storage is located in each classroom. Classroom 4 also has a plan in place because this medication travels back and forth from home to school. The medication tote bag will be handed to the teacher upon arrival the teacher will then hang it on a hook on the closet door out of the reach of children. If the parent does not see the teacher the parent will hang the bag on the hook.
