Staff shall only administer medication with: (1) A valid and current prescription or signed and dated written instructions for administering the medication from the child’s physician; and (2) Signed and dated written permission from the parent.
The licensing coordinator observed prescription medication for children in care, and the provider stated that they do not obtain a parent's signature authorizing them to administer prescription medications to children in the program. Corrective action: Program has created a Google Form for parents/guardians to complete when prescription medication is given to program to administer. The Form includes the child's name, prescription name, dosage information, the parent's name, date the prescription was given to program staff and date returned to the family.
