Programs shall not accept any prescription medications that do not include a prescription label or medication order from a licensed professional.

The licensing coordinator observed that in the Appleseeds classroom, there was an Auvi-Q epinephrine injector for an enrolled child, and there was no prescription label or medication order from a licensed professional for the medication. Corrective action: We will ask parents to bring in the original packaging with the pharmacy label. If that is not available, we will ask for the medication plan form to be filled out by the child's doctor. If parents bring in meds without either of those items, we will refuse it and send it back home until the proper paperwork is supplied. Depending on the severity of the allergy/medical need, we may ask that their child not attend until necessary meds & documentation are presented.

The licensing coordinator observed that in the bathroom of the Appleseeds classroom, there was an expired Auvi-Q epinephrine injector belonging to an enrolled child which had not been returned to the child's parent or guardian as required. Corrective action: Parents were notified on 4/23/25 that the medication was expired. Going forward, we will begin recording medication expiration dates when medications are brought in at the beginning of the school year. Also a staff member has been designated to check all medication expiration dates 1x per month.

The licensing coordinator's observation of a screwdriver in an unlocked drawer 35 inches from the floor in the Appleseeds classroom. The licensing coordinator moved the screwdriver out of reach, since the children and teachers were outside at the time of the visit. Corrective action: A staff member has been designated to check all drawer latches 1x per month. Teachers will continue to report faulty latches as needed. If a latch is found to be faulty, OOR items will be moved until latches can be repaired.

The center director, site director, family child care provider, and all staff used to meet staff to child ratios shall: (1) Be certified in pediatric cardiopulmonary resuscitation (CPR) and first aid within 90 days of the first date of employment; (2) Obtain certification in (m)(1) above by the American Red Cross, American Heart Association, Emergency Care and Safety Institute, National Safety Council, or other nationally recognized organization; and (3) Maintain current certifications required in (m)(1) above.

The licensing coordinator's review of five staff files revealed that certification in pediatric CPR and First Aid was expired for one staff. Corrective action: Affected staff member will renew their certification ASAP. We mistakenly thought the certification was good for 3 years, but it was only good for 2. Going forward all staff will re-certify 1-3 months before expiration.
