A checklist was used to note the requirements I monitored today. This facility currently operates with Five Star license issued on July 9,2019. Upon my arrival to the facility, I was greeted by Ms. A. Hogan, the lead teacher/owner. Ms. Hogan assisted me during my visit today. All indoor spaces used use by the children was observed today. I was unable to monitor the playground due to the weather All required items were posted. All medication and hazardous items were out of the children's reach. Seven (7) children were present during today’s visit. I observed the children playing on the floor with available toys, making a gingerbread house, washing their hands in preparation for lunch, eating lunch and napping. A sanitation inspection was conducted on December 10,2024. This facility received a superior rating with two (2) demerits. A fire inspection was conducted on January 30,2024. The original will be mailed to Raleigh with this documentation. Staff and children files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. This facility do not transport therefore I did not monitor transportation requirements. Three (3) violations were cited during today’s visit. Violation Number Comment Rule 125 Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. 10A NCAC 09 .0302(d)(4) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. Monthly playground inspections were not available for review. .0605(q) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. Two new staff, who had contact with children, did not have documentation they receive at least 16 hrs. orientation within first 6 weeks. The hours were not complete on the form. .1101(a) The above violations must be corrected immediately, and a letter must be email to me no later than December 25,2024 stating how each item was corrected. My email address is wytina.blackmon@dhhs.nc.gov. If I do not receive your letter by December 25,2024 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. Consultation: I discussed with Ms. Hogan that the staff members need in-service training hours, It-SIDS, health and safety training, criminal rechecks, CPR and First Aid trainings by specific dates. Please refer to the staff/training worksheet. I need the certificate for your Health and Safety training by January 31,2025. If I do not receive the certificate, then I will cite a violation. Technical Assistance: Today, I discussed with Ms. Hogan the requirements for the incident form and log. This report shall be signed by the person completing it and by the parent, a copy given to the parent or the parent declining a copy and the report maintained in the child's file. You must complete the form with every incident. This will allow you to have documentation in case something serious happens form the incident. You will only need to send the form to your consultant if the child receives medical attention. You will have seven days to submit the form. Today you showed me your electronic documentation of incidents. I explained to you that you will need to have the parent's signature as well as all the information that is on the DCDEE state form is required to be on what form you create or electronic system you use. You stated you understood and you would still use your electronic system. Rated License Assessment: I also discussed with Ms. Hogan the Rated license assessment process the hold harmless has been extended until the new QRIS is implemented. This means that you are not required to go any further with a rated license assessment unless you want to proceed. Education: WORKS: Please ensure that all new staff member’s education information is current in the W.O.R.K.S. database. Each staff member must register in the WORKS system as soon as they are hired at this location. We reviewed the instructions on the DCDEE website. Education evaluations can take some time to process so beginning the process early will help to ensure we have the accurate information needed to ensure your staff is meeting minimum education requirements. There are two other education options. One options you may also want to consider having staff members complete the Early Educator Certification process to assist in getting higher education evaluations completed if their degree is not in early childhood education. Early Educator information is available at www.ncicdp.org. The other options is to contact Deja Lewis Education& Training program Administration Manager with Generation-Ed. The facility has an 18-month compliance history of 96%. The last annual compliance visit was documented on 1/12/24. The facility’s corporation status was verified current on the NC Department of the Secretary of State today, 6/10/24. Upon arrival, I was greeted by Jasmine Blanks. Kaylah Rowland arrived during the visit and assisted me with program records. Jordan Miller was also present and was providing care for children 0-1. Mrs. Hogan was not present during today’s visit. There were 12 children present today, ranging in age from 0-3 years old. Children were observed using the restroom and napping. Indoor and outdoor spaces were monitored and there were no hazards observed today. Fire drill documentation was posted, however there was no information for March and April drills. Other emergency drills were not documented quarterly; the last documented drill was January 10, 2024. A sanitation inspection was completed on 11/3/23 and the facility received a Superior rating. Outdoor inspections could not be located during the visit. Required items were posted. There were three items of non-compliance observed today. Violation Number Comment Rule 805 Fire drills were not practiced monthly and/or the drill record was incomplete. The fire drill record was incomplete for the months of March and April. .0604(t); .0302(d)(5) 1041 Prior to employment a Criminal Background Check was not completed. There was not a qualification letter nor information online for J. Miller and J. Blanks. G.S. 110-90.2(b) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last documented drill was January 10, 2024. .0604(u);.0302(d)(8) Violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 6/24/24, describing how and when the violation(s) were corrected and how compliance will be maintained in the future. Please remember your letter should include facility name, facility ID#, and original signature. Jordan Miller and Jasmine Blanks did not have qualification letters on file during today’s visit. I reviewed the online system and neither had been qualified by DCDEE. They can not be on-site until criminal background checks are completed by DCDEE and they receive qualification letters. I explained this to both of them and to Ms. Rowland who was in contact with Mrs. Hogan. Thank you for your time today. If you have any questions or concerns, please feel free to contact me.
