This facility currently operates with Five Star license issued on May 23,2023. The last annual compliance visit March 11,2026. I arrived at the facility to investigate the allegation received in the complaint report. Upon my arrival, I was greeted J. Dungee, one of the administrators. I informed her the purpose of today’s visit. Ms. Dungee gave me the necessary paperwork I need to complete my visit. I observed and documented the attendance. One hundred and eighteen (118) children were present during today’s visit. I observed the children playing on the floor with available toys, being fed, finishing up eating lunch and napping. I conducted interviews with the administrative staff and other staff present. The allegation was confirmed as an ongoing issue that this facility has been dealing with in several spaces in the building especially in the cafeteria and kitchen areas. The issue has been addressed. During today’s complaint visit, Ms. Dungee informed me of the steps taken to correct the issue. She showed me the receipt where the facility has had pest control at the facility. She also has another pest control request to be scheduled, but she is waiting for approval from the facility’s management personnel. Limited monitoring of childcare requirements occurred during today's visit. Based on information gathered from observations and staff interviews, violation of child care requirements mentioned in the allegation was confirmed. One (1) violation was cited during today’s visit. Violation Number Comment Rule 899 The child care operator did not comply with all State laws, federal laws and/or local ordinances that pertain to child health, safety, and welfare, as required by General Statute 110-91. The facility did not comply with sanitation requirements. Pests shall be eliminated from the child care center 15A NCAC 18A .2831 GS 110-91 The above violation must be corrected immediately, and a letter must be emailed to me no later than April 13,2026 stating how the one item was corrected. If I do not receive your letter by April 13,2026 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. I explained to Ms. Dungee Child Care Rule 2203 does require an Administrative Action be considered and allegations are confirmed, however the steps taken to correct the violation will be considered in this process. A checklist was used to note the requirements I monitored today. This facility currently operates with Five Star license issued on May 24,2023. Upon my arrival, I was greeted by Ms. J. Dungee, the director. I informed Ms. Dungee the purpose of today’s visit. Ms. Dungee gave me the necessary paperwork I needed to complete today’s visit. Ms. Dungee accompanied me as I observed and documented the attendance. One hundred ninety (190) children were present during today’s visit. They were observed being fed, sleeping, being diapered, playing outside on the playground, playing in centers, washing their hands in preparation for lunch, eating lunch, napping, waking up from napping, washing their hands for snack, arriving from school and departing. All indoor and outdoor spaces used by the children were observed today. All required items were posted. All medication and hazardous items were out of the children's reach. A sanitation inspection was conducted on August 26,2025. This facility received a superior rating with nine (9) demerits. A fire inspection was conducted on June 10,2025. The original will be mailed to Raleigh with this documentation. Staff and children’s files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. I monitored two fifteen passenger buses this facility uses to transport the children from school and field trips. The insurance policy was current for the vehicles. The vehicles met the transportation requirements during today's visit. I checked the NC Department of the Secretary of State for your corporation status. Your corporation status is active and current as of March 11,2026. Four (4) violations were cited during today’s visit. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. A safe outdoor environment was not provided for the children. There were gaps under the fences that are entrapment hazards. Also the rubber surfacing has a pice torn that is a tripping hazard. 10A NCAC 09 .0601(a) 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. One staff did not complted the required number of in-service training. .1103(a) 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. Identifying information, including photograph, was not in the vehicle for nine children being transported. 10A NCAC 09 .1003(d) 1314 Emergency information did not name childs health care professional. I informed Ms. Dungee she will need to contact the corporate owner and inform them they will need to choose another path until this facility completes the accreditation process. We discuss the other two Pathways and Pathway 1 would be a better choice for this facility. WORKS: ensure the staff’s education information is in the W.O.R.K.S. database. Please review the steps for submitting education to our W.O.R.K.S on our website. Education evaluations can take some time to process so beginning the process early will help to ensure we have the accurate information needed to ensure your staff meet minimum education requirements. Clean Classrooms for Carolina Kids This facility completed the lead water on August 17,2025. This test should be completed every three years. The facility has also enrolled in the lead paint and asbestos training. You will need to complete these trainings and request the kit by March 25, 2026. I observed the children arriving, playing on the floor with available toys, playing in centers, washing their hands in preparation for lunch, eating lunch, napping, waking up from napping, washing their hands in preparation for snack, eating snack and departing. A sanitation inspection was conducted on December 3, 2024. This facility received a superior rating with four (4) demerits. A fire inspection was conducted on October 3,2024. The original will be mailed to Raleigh with this documentation. Staff and children’s files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. I will email you the section of the rule with this documentation. There are several children’s topical creams, and permissions to admission form and medical action plans and expirations of medications are due by specific dates. Please have each teacher check their forms monthly to ensure the authorization expires. Rated License Assessment: I discussed it with Ms. Dungee the Rated license assessment process the hold harmless has been extended until the new QRIS is implemented. This means that you are not required to go any further with a rated license assessment unless you want to proceed. I explained to Ms. Dungee that there is a new process that is still being developed. Beginning on February 1, 2025, child care facilities will use the Environment Rating Scale – Third Edition for family child care homes, infant/toddler classrooms and preschool classrooms, if requesting a program assessment or reassessment. You can visit ncrlap.org for any upcoming training courses for ITERS-3 and ECERS-3. WORKS: Please ensure that all new staff member’s education information is current in the W.O.R.K.S. database.
