A checklist was used to note the requirements I monitored today. This facility currently operates with Three Star license issued on September 15,2025. Upon my arrival to the facility, I was greeted by one of the lead teachers. She informed me that Ms. G. Caine, the director, was in a classroom. I went to the classroom where Ms. Caine was located and informed her the purpose of today’s visit. Ms. Caine gave me the necessary paperwork to complete my visit. All indoor and outdoor spaces used by the children were observed today. All required items were posted. All medication and hazardous items were out of the children's reach. Thirty-six (36) children were present during today’s visit. I observed the children playing on the floor with available toys, playing in centers, playing outside on the playground, washing their hands in preparation for lunch, eating lunch, and napping, A sanitation inspection was conducted on February 19,2026. This facility received a superior rating with fourteen (14) demerits. A fire inspection was conducted on May 29,2025. The original will be mailed to Raleigh with documentation. Staff and children’s files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. I checked the NC Department of the Secretary of State for your corporation status. Your corporation status is active and current as of March 31,2026. Six (6) violations were cited during today’s visit. Violation Number Comment Rule 707 Equipment and furnishings not meeting the requirements outlined in child care .0601(b) and (c) were not removed or made inaccessible. There were broken toys. There were two cars, rusty metal truck, push toys, and musical toys. These were all removed during the visit to correct the violation. .0601(d) 807 A safe indoor and outdoor environment was not provided for the children. A safe outdoor environment was not provided for the children. There were two cables in ground creating tripping hazards. Also there are holes in the side of the building and sharps rocks that are safety hazards. 10A NCAC 09 .0601(a) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. There were balls in a plastic bag accessible to the children on the playground. This was corrected during the visit. .0604(q) 1125 Before children were transported, written permission from a parent was not obtained that included when and where the child was to be transported, expected time of departure and arrival, and the transportation provider. Before eleven children were transported, written permission from their parents were not obtained that included when and where the child was to be transported, expected time of departure and arrival, and the transportation provider. .1003(i)(j) 1127 For routine transport of children to and from the center, staff did not have a list of the children being transported. There was not a list of the children being transported at the center. 10A NCAC 09 .1003(l) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The facility administrator has not completed the ABCMS provider portal adding the new employees. G.S. 110-90.2 & .2703(r) The above violations must be corrected immediately, and a letter must be emailed to me no later than April 14,2026 stating how each item was corrected. If I do not receive your letter by April 14,2026 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. Consultation: I discussed with Ms. Caine that the staff members need training hours, health and safety CPR and First Aid trainings by specific dates. Please refer to the staff/training worksheet. ABCMS During today’s visit, I discussed Ms. Caine that all staff must be listed under the facility in the ABCMS provider portal. Here is the portal the ABCMS Provider Portal and required Moodle training. I am conducting a training on Saturday April 11,2026 at 10:00am at Generation Ed. Clean Classrooms for Carolina Kids This facility completed the lead water on November 15,2024. This test should be completed every three years. The facility has also enrolled in the lead paint and asbestos training. You will need to complete these trainings and request the kit by Aprill 14, 2026. Failure to complete the training by this specific date then a violation will be added to this visit. Thank you for your time. If the operator fails to correct any documented violations within the established time period, the Division of Child Development and Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000).
