Elizabeth Childress, Child Care Consultant, accompanied me on the visit. You, Mrs. Michelle Millard, owner/operator, assisted us with the visit. Your program currently operates with a Five Star license effective 12/03/2019. The license was posted, and the restrictions were in compliance. Compliance score as of 10/01/2025 was 100%. A walk-through of the home was completed today, and all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Five children were present during today’s visit. Children throughout the home were participating in free play in activity areas . The last Fire Inspection was completed 12/18/2024 and remains current. An Operator Statement of Responsibility and a Verification of Required Information for Operators and Additional Caregivers was completed on 03/04/25. One violation was observed today. Violation Number Comment Rule 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance). The providers criminal background check expired 09/25/2025. G.S. 110-90.2 & .2703(n) & (o) To maintain compliance with North Carolina child care laws and rules, the above violation must be corrected immediately. Please send me a correction letter by email stating how the violations were corrected. It must be submitted tome by October 16, 2025. Include the following information in your correction letter: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number, the date the violation was corrected, information regarding how the violation was corrected. We discussed the following: Criminal Background Checks for you and Michael Millard. They expired 09/25/2025. We provided assistance regarding how to make an appointment for a new CBC. QRIS Pathways to the Stars: the Conversation Template was completed. Currently, you are considering Pathway 2. According to the Clean Water for Kids website, enrollment for lead-based paint and asbestos has been started but will need to be completed.
