I was greeted by Group Leader Ms. Pamela Ragin, and I explained the purpose of the visit. The Site Coordinator, Ms. Tamara Hunter, arrived shortly after my arrival. Upon arrival, I observed Group Leaders setting up activity centers. The facility had a Five-Star License issued February 27, 2022, and an eighteen-month compliance history score of ninety-seven (97) percent prior to today’s visit. I observed the children arrive at the ASEP Program at 3:15 p.m. There were five (5) groups of children observed with five (5) Group Leaders. The children were observed handwashing and preparing for an afternoon snack. The following snack items were observed posted and were served to the children: Unflavored skim milk/ one (1) percent milk, or one-hundred percent Juice six (6) ounces, Yogurt, and three (3) count Graham Crackers. I observed the following items posted: License, No Smoking Sign, Summary of NC Law, Menu, a current Activity Plan (PBIS ASEP Activity Plan), ASEP Schedule, First Aid Poster, the Emergency Medical Care Plan, and the CMS Crisis Plan. There were five (5) activity areas that were available: Homework, Arts/Crafts, Blocks, Math, Reading, and Writing. The children's records were reviewed, and compliance was met. Staff records were reviewed with the Staff and Training Worksheet. A veteran staff file was reviewed and compliance was met, and a new staff members file was reviewed; a violations was cited for Recognizing and Responding to Suspicions of Child Maltreatment Training. The roster was observed complete in the Automated Background Check System (ABCMS). I did not observe any hazardous products and bio contaminants improperly stored; compliance was met. There were not any children with medication, and children with allergies were listed. One (1) violation was cited. Violation Number Comment Rule 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. A staff (B.G.) has not completed Recognizing and Responding to Suspicions of Child Maltreatment within ninety (90) days of employment. .1102(g) -Scales will be requested in early September 2026. -The oldest child present during the visit was a thirteen (13) year-old child, which is not reflected on the Enrollment Worksheet; the child is included in the number of children present. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before April 13, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. The implementation plan will allow time for administrators/operators to learn and understand the different pathways and the changes within our system. We encourage you to visit the QRIS Modernization page on the DCDEE website to learn more now. -The DCDEE team looks forward to working with child care facilities across the state to transition to the new QRIS system, also known as Pathways to the Stars, located in Section .3200 of the Child Care Rules. These third editions come with a spiral binding at the top, replacing the current revised editions. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. •Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. •Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. As part of ongoing efforts to protect state systems and data, the N.C. Department of Information Technology will be changing the minimum length of NCID passwords from eight to 14 characters. The change will affect state agency staff as well as external users like citizens, providers, and state and local county staff. There is no action you need to take on or before February 23rd. I was greeted by South Academy of International Languages In School Suspension Staff Danielle Cloutier, and I explained the purpose of the visit. The facility had a Five-Star License issued February 27, 2022, and an eighteen-month compliance history score of 98% prior to today’s visit. I was informed by school office staff and Ms. Cloutier that the ASEP Coordinator is absent for the day. Ms. Cloutier and two (2) Assistant Principals, Ms. Cassandra Mayo and Ms. Victoria Perez were present with the children. The children were observed eating snacks. After snack, I observed activities that CMS staff provided to the children, such as a variety of games, and puzzles, and I observed some children completing homework assignments. The ASEP Coordinator, Ms. Tamara Hunter, arrived later during the visit. Two (2) CMS ASEP staff arrived during the visit, ASEP Program Assistant Kimmie Howard, and ASEP Substitute Remedios Martin. Ms. Howard provided a copy of her Criminal Qualifying Letter and a Ms. Howard provided a copy of her CPR/First Aid. Ms. Martin did not have a copy of her CBC letter; Ms. Mayo and Ms. Perez did not have CBC letter. A violation was cited for staff not having CBC letters. I observed the following items posted: License, No Smoking Sign, Summary of NC Law, Menu, a current Activity Plan (PBIS ASEP Activity Plan), ASEP Schedule, First Aid Poster, the Emergency Medical Care Plan, and the CMS Crisis Plan. I observed a Fire Drill that was conducted on August 30, 2023. The children's records were reviewed and compliance was met. Staff records was reviewed with the Staff and Training Worksheet and compliance was met; ten percent (10%) of records reviewed I did not observe and hazardous products and bio contaminants improperly stored; compliance was met. There were not any children with medication, and children with allergies was listed. The outdoor play area was observed and the outdoor play area was found in good repair. The Sanitation Inspection was conducted January 11, 2023 with a "superior" rating and zero (0) demerits. The Fire Inspection was completed January 30, 2023. The following Technical Assistance (TA) was provided: -Continue to review emails from the Division of Child Development and Early Education (DCDEE) and from your assigned child care consultant. -House Bill 103: Effective July 11, 2022, individuals have five (5) years to renew their criminal record check instead of every three (3) years. -On June 12th, Governor Cooper signed Senate Bill 291, extending the Child Care Hold Harmless legislation until June 30, 2024, and requiring the North Carolina Child Care Commission to make recommendations for modernizing the state’s Quality Rating and Improvement System. -NCDHHS Children’s Environmental Health has conducted a series of virtual trainings on the recently re-adopted Child Care Sanitation Rules, effective July 1, 2023. The rules have been approved but have not yet been updated in the Administrative Code. Once updated, notification will be sent out via listserv. -The North Carolina Child Care Commission and the North Carolina Department of Health and Human Services Division of Child Development and Early Education are collaborating to modernize NC’s child care quality rating and improvement system (QRIS), commonly known as the Star Rated License. We are developing a plan that will be reviewed by the legislature in Spring 2024. During the next few months, we are collecting information from parents, teachers, administrators, operators, and partners to consider in the plan. -If you are unable to attend these sessions, you may also complete an online survey or write to DCDEE_QRIS@dhhs.nc.gov. -Health Safety Training is due to be completed by the end of September 2023. I was greeted by South Academy of International Languages In School Suspension Staff Danielle Cloutier, and I explained the purpose of the visit. The facility had a Five-Star License issued February 27, 2022, and an eighteen-month compliance history score of 98% prior to today’s visit. I was informed by school office staff and Ms. Cloutier that the ASEP Coordinator is absent for the day. Ms. Cloutier and two (2) Assistant Principals, Ms. Cassandra Mayo and Ms. Victoria Perez were present with the children. The children were observed eating snacks. After snack, I observed activities that CMS staff provided to the children, such as a variety of games, and puzzles, and I observed some children completing homework assignments. The ASEP Coordinator, Ms. Tamara Hunter, arrived later during the visit. Two (2) CMS ASEP staff arrived during the visit, ASEP Program Assistant Kimmie Howard, and ASEP Substitute Remedios Martin. Ms. Howard provided a copy of her Criminal Qualifying Letter and a Ms. Howard provided a copy of her CPR/First Aid. Ms. Martin did not have a copy of her CBC letter; Ms. Mayo and Ms. Perez did not have CBC letter. A violation was cited for staff not having CBC letters. I observed the following items posted: License, No Smoking Sign, Summary of NC Law, Menu, a current Activity Plan (PBIS ASEP Activity Plan), ASEP Schedule, First Aid Poster, the Emergency Medical Care Plan, and the CMS Crisis Plan. I observed a Fire Drill that was conducted on August 30, 2023. The children's records were reviewed and compliance was met. Staff records was reviewed with the Staff and Training Worksheet and compliance was met; ten percent (10%) of records reviewed I did not observe and hazardous products and bio contaminants improperly stored; compliance was met. There were not any children with medication, and children with allergies was listed. The outdoor play area was observed and the outdoor play area was found in good repair. The Sanitation Inspection was conducted January 11, 2023 with a "superior" rating and zero (0) demerits. The Fire Inspection was completed January 30, 2023. Three (3) violations were cited during the visit. Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. Prior Criminal Background Checks were not completed for Assistant Principals (C.M.) and (V.P.) and CMS Staff (D.C.). G.S. 110-90.2(b) 1756 Enhanced staff/child ratios and group sizes were not met.
