I was greeted by Owner/operator, Ms. Cynthia Thomas, and I explained the purpose of the visit. An indoor/outdoor walkthrough of the facility was not completed; an open violation was monitored for compliance. The following violation documented during the December 15, 2025, visit was monitored for compliance during this visit: (#1847) – A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The facility's roster was incomplete in ABCMS. A violation was cited for #1847 (Repeat Violation (twice)), because the roster was incomplete in ABCMS. One (1) violation was cited. Violation Number Comment Rule 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. e facility's roster was incomplete in ABCMS. G.S. 110-90.2 & .2703(r) -Ms. Thomas has stated that the training has been completed in Moodle, and she was informed that the DocuSign document must be completed. Ms. Thomas stated that she has attempted to login Moodle to complete the DocuSign, however, she stated that the Moodle page stated that the system will be down from June 6th to June 8th 2026. During the visit Ms. Thomas contacted the Moodle Helpline, and the assistance line stated that if you are having technical difficulty, you must submit an email for technical assistance; Ms. Thomas sent an email during the visit. -It was verified during the visit that Ms. Thomas could not login to Moodle. -Failure to correction the violation(s) is failure to maintain compliance; may result in an administrative action. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before June 22, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. •Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. I was greeted by Owner/Operator Ms. Cynthia Thomas, and I explained the purpose of the visit. Ms. Thomas daughter was present upon arrival, and Ms. Thomas daughter left; was not present during the visit. The facility has a Three-Star License issued August 23, 2024. The sanitation inspection was completed November 19, 2025 with a demerit score of six (6). The Emergency Preparedness and Response Plan (EPR) was reviewed; compliance was met. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-six (96) percent as of February 5, 2026. The NC Secretary of State website was reviewed on February 5th and 6th, 2025, and Genesis Family Child Care Home LLC was listed as corporation status is in administrative dissolution. Upon arrival, there was one (1) toddler present. Another toddler arrived during the visit. Upon the toddlers arrival I observed the parent assist the child with handwashing. I observed Ms. Thomas reading a book (Clifford the Big Red Dog) to the children. Nurturing tones were heard when Ms. Thomas spoke to the children. The facility was monitored today with the Family Child Care Home Monitoring Checklist and the Family Child Care Home Item Number Listing. A copy of the checklist was provided to Ms. Thomas after the visit. The following were observed posted: Current License, NC Summary of Child Care Law, Schedule, First Aid Sheet, Activity Plan, the Emergency Medical Care Plan, and Emergency Numbers; compliance was met. The Incident Log was reviewed; compliance was met. Monthly Fire Drills, Shelter-in-Place/Lockdown Drills were reviewed; compliance was met. Playground Inspections were observed and compliance was met. Ms. Thomas has two (2) children enrolled on second shift. There are no children enrolled on third shift. Ms. Thomas is using an approved curriculum, Creative Curriculum. The daily attendance and arrival and departure times were reviewed and met compliance. The child care facility, including the bathroom, child care room, kitchen, and outdoor play area was monitored during the visit. Soap and toilet paper were available in the bathroom for use. The toys and equipment were in good repair. In the kitchen, there was a working fire extinguisher, working thermometer in the refrigerator, and smoke detectors. Ms. Thomas file was monitored and observed for ongoing training hours Health Questionnaire, Emergency Information, First Aid and CPR, and Criminal Qualifying Letter; compliance was met. No one else resides in the home with Ms. Thomas. The roster was incomplete in the Automated Background Check System (ABCMS) and a violation was cited. The children's files were monitored today. A violation was cited for the enrollment dates for the discipline policy, a violation was cited for the enrollment date on the shaken baby syndrome and abusive head trauma policy, and a violation was cited for a child's file that did not contain the shaken baby syndrome and abusive head trauma policy. Ms. Thomas pet was up to date on vaccinations; compliance was met. There are no children enrolled with allergies, and there are no children on medication or emergency medication. The outdoor play area was observed, and there were no hazards observed. The outdoor play area was clean and the toys were in good repair; balls, and other toys are available for outdoor play. I was greeted by Owner/Operator Ms. Cynthia Thomas, and I explained the purpose of the visit. Ms. Thomas daughter was present upon arrival, and Ms. Thomas daughter left; was not present during the visit. The facility has a Three-Star License issued August 23, 2024. The sanitation inspection was completed November 19, 2025 with a demerit score of six (6). The Emergency Preparedness and Response Plan (EPR) was reviewed; compliance was met. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-six (96) percent as of February 5, 2026. The NC Secretary of State website was reviewed on February 5th and 6th, 2025, and Genesis Family Child Care Home LLC was listed as corporation status is in administrative dissolution. Upon arrival, there was one (1) toddler present. Another toddler arrived during the visit. Upon the toddlers arrival I observed the parent assist the child with handwashing. I observed Ms. Thomas reading a book (Clifford the Big Red Dog) to the children. Nurturing tones were heard when Ms. Thomas spoke to the children. The facility was monitored today with the Family Child Care Home Monitoring Checklist and the Family Child Care Home Item Number Listing. A copy of the checklist was provided to Ms. Thomas after the visit. The following were observed posted: Current License, NC Summary of Child Care Law, Schedule, First Aid Sheet, Activity Plan, the Emergency Medical Care Plan, and Emergency Numbers; compliance was met. The Incident Log was reviewed; compliance was met. Monthly Fire Drills, Shelter-in-Place/Lockdown Drills were reviewed; compliance was met. Playground Inspections were observed and compliance was met. Ms. Thomas has two (2) children enrolled on second shift. There are no children enrolled on third shift. Ms. Thomas is using an approved curriculum, Creative Curriculum. The daily attendance and arrival and departure times were reviewed and met compliance. The child care facility, including the bathroom, child care room, kitchen, and outdoor play area was monitored during the visit. Soap and toilet paper were available in the bathroom for use. The toys and equipment were in good repair. In the kitchen, there was a working fire extinguisher, working thermometer in the refrigerator, and smoke detectors. Ms. Thomas file was monitored and observed for ongoing training hours Health Questionnaire, Emergency Information, First Aid and CPR, and Criminal Qualifying Letter; compliance was met. No one else resides in the home with Ms. Thomas. The roster was incomplete in the Automated Background Check System (ABCMS) and a violation was cited. The children's files were monitored today. A violation was cited for the enrollment dates for the discipline policy, a violation was cited for the enrollment date on the shaken baby syndrome and abusive head trauma policy, and a violation was cited for a child's file that did not contain the shaken baby syndrome and abusive head trauma policy. Ms. Thomas pet was up to date on vaccinations; compliance was met. There are no children enrolled with allergies, and there are no children on medication or emergency medication. I was greeted by Owner/Operator Ms. Cynthia Thomas, and I explained the purpose of the visit. Ms. Thomas daughter and son were present in the home during the visit. The facility has a Three-Star License issued August 23, 2024. The sanitation inspection was completed December 5, 2024, with a demerit score of zero (0). The Emergency Preparedness and Response Plan (EPR) was reviewed; the plan was updated September 17, 2024. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-seven (97) percent as of February 9, 2025. The NC Secretary of State website was reviewed on February 10, 2025, and Genesis Family Child Care Home LLC was listed as current-not active; a violation was cited. Upon arrival, there were not children present. The facility was monitored today with the Family Child Care Home Monitoring Checklist and the 2025 Family Child Care Home Item Number Listing. A copy of the checklist was provided to Ms. Thomas after the visit. The following was observed posted: Current License, NC Summary of Child Care Law, Schedule, First Aid Sheet, Activity Plan, the Emergency Medical Care Plan, and Emergency Numbers; compliance was met. The Incident Log was reviewed. Monthly Fire Drills, Shelter-in-Place/Lockdown Drills were reviewed; a violation was cited. Playground Inspections were observed and compliance was met. Ms. Thomas has one (1) child enrolled on second shift. There are no children enrolled on third shift. Ms. Thomas has adequate space and bedding for child(ren) that enroll on second shift; mattresses are available night time care for; compliance met. Ms. Thomas is using an approved curriculum, Creative Curriculum. The daily attendance and arrival and departure times were reviewed and met compliance. The child care facility, including the bathroom, child care room, kitchen, and outdoor play area was monitored during the visit. Soap and toilet paper were available in the bathroom for use. The toys and equipment were in good repair. In the kitchen, there was a working fire extinguisher, working thermometer in the refrigerator, and smoke detectors. A violation was cited for a hazardous item in the bathroom. Ms. Thomas file was monitored and observed for Health Questionnaire, Emergency Information, First Aid and CPR, and Criminal Qualifying Letter; a violation was cited for the Health Questionnaire. No one else resides in the home with Ms. Thomas. The child's files was monitored today, and the child's file met compliance. Ms. Thomas pet was up to date on vaccinations. A violation was cited for a pet on the premises vaccination records. There are no children enrolled with allergies, and there are no children on medication or emergency medication. The outdoor play area was observed, and there were no hazards observed. The outdoor play area was clean and the toys were in good repair; balls, and other toys are available for outdoor play. Ms. Thomas does not provide transportation and a vehicle was available in case of an emergency Four (4) violations were cited. The shelter-in-place drill was not completed in August 2025. .1719(a )(16) & .1721(e )(7) A follow-up visit will be conducted in the near future to monitor child(ren) present. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before February 24, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. They must keep a record that includes the date of each drill, time of day, the length of time to reach the designated location and the signature of the person conducting the drill, Child Care Rule .0604(u). -Ongoing training must be documented on the ongoing training log (Child Care Rule 10A NCAC 09 .1103. -CBC Provider Portal Technical Assistance & Notification to the Division of New Hires or Residents Technical Assistance Guidance: • As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. • The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. •Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. The program also reviews child care facilities’ documentation and other information to determine and coordinate any steps needed to identify and eliminate lead-based paint and asbestos hazards (10A NCAC 41C.1001-1007). Licensed child care centers operating within a public school (i.e., afterschool programs, Head Starts, Pre-K, etc.) should coordinate with school staff to complete the lead in water section of the program so the entire school can be tested at once. -Starting on February 1, 2025, the ECERS-3, ITERS-3, and FCCERS-3—also known as the "3s"—will be used for DCDEE environment rating scale assessments. The DCDEE and the North Carolina Rated License Assessment Project (NCRLAP) are collaboratively working on preparations for the transition to the third editions. These third editions come with a spiral binding at the top, replacing the current revised editions.
