During the complaint inspection, it was found that none of the rooms had accurate staffing patterns posted. LS is aware that there have been some very recent changes in staff. Please provide a copy of accurate staffing patterns for each room to OCC for the correction of this non-compliance.

It was determined by the Center that an Aide in the 2-3 year old classroom caused injuries to the left forearm and right underarm of a 3 year old child during nap time. The Center has a recorded video of the staff member being aggressive with the child, pulling her onto and off of her cot, holding her forcefully in her lap as well as forcefully holding her down on her cot. The staff member reported to the Center that she accidentally caused the injuries when she pulled the child out from underneath a table. However, this was not able to be seen in the video. This account may not be a logical explanation of the injuries, especially since the child came out from under the table on her own and away from the direction of the staff member. Despite the mode of injury, the staff member was inappropriate and excessively forceful with the child as evidenced in the video. The Center acted appropriately by firing that staff member the same day as the incident as soon as they were made aware of what occurred.
Even though the Center saw injuries on a 3 year old child from the actions of a staff member, no one reported the incident to OCC or to CPS or to law enforcement. The Center did, however, fire the staff member that same day as the incident and informed the parent. Please write a letter of correction stating how you will become in compliance with this regulation in the future and submit it to OCC for the correction of this non-compliance.
Per the recorded video, the identified staff person used inappropriate discipline by struggling with the child to retrieve her from underneath the table. That same staff member also slid the child across the cot while using a great amount of force. That staff member also forcibly held the child in her lap and then down on the cot. This exhibits inappropriate discipline by the staff member which is against regulation and against the standards and practices set forth by the Center.
During the inspection, the LS noticed that four out of the five posted activity plans in the Caterpillar A Room have not been updated since June of this year. They must be updated, at least, every 3 months. An activity plan was also missing for the sixth child enrolled in the room. Please have the parent of the sixth child complete an activity plan for that child and send a copy of it to OCC. Have the other parents of the four children update their respective child's plan. Send a copy of those updated plans to OCC for the correction of this non-compliance.
When asked about inaccurate staffing patterns during the inspection, the Center reported that they are currently training staff and she will then see where their best fit will be before changing staffing patterns. However, these staff are not included on the substitute logs. In addition, a substitute log has not be completed for the Butterfly room since 05/01/2025, but they are continuing to use substitutes. Please provide proof to OCC of utilizing the sub log for all substitutions on the current staffing patterns for the correction of this non-compliance.
When going over the associated party list with the Center, there were several staff who no longer work at the Center, but a 1203 form to delete them had not been submitted. During the inspection, a 1203 was submitted by the Center for all of the staff no longer working here or those that are being changed to a substitute. Therefore, this non-compliance was corrected on-site.
During the inspection, it was observed that an Aide was alone in a room with 2 two year old children and 3 three year old children. It was also observed that a newly hired Aide was briefly alone with a group of four children aged 19 months old and 3 two year old children. Later, the only teacher on that floor at that time, joined that Aide in her classroom. That then left the room that the teacher was in with 2 Aides and 9 children aged 18 months, 19 months and 5 two year old children. Later in the day, another teacher returned to one of the rooms that had only Aides in the classroom. The Center states that this an unusual circumstance because one of the teachers was out of the classroom because she was assisting on a field trip day. Please submit variance requests to OCC for the Aides who may be close to qualifying as a teacher. Please write a letter of correction stating how the Center will become in compliance with this regulation in the future.
During a review of the emergency disaster plan, it was found that it was last reviewed and updated in June of 2023. The plan must be updated at least on an annual basis. During the inspection, the Center reviewed the plan, initialed and dated it for the review. Therefore, this non-compliance was corrected on-site.
During the inspection, it was discovered that six staff no longer work at the Center. OCC was not aware of those six staff no longer working at the Center or having never started at the Center after being approved to work at the Center. The Center is reminded that OCC needs to receive written notification of the ending of employment within five working days. This non-compliance was corrected on-site by the Center providing a 1203 deleting the six staff members.
During a review of staff records, it was found that a staff member has not had a medical evaluation performed since 08/23/2011. A medical evaluation must be completed on OCC's medical at least every five years. The Center sent her to get a medical completed on the OCC form today as she was already going to the doctor for an appointment. At the end of the day, this staff member returned to the Center with a completed medical on the OCC form which was viewed by the Licensing Specialist. Therefore, this non-compliance was corrected on-site.
A child care teacher has only completed 1.5 hours out of her required 12 hours of training for her 06/2023-06/2024 training year. She reports that she has completed training, but cannot produce the training certificate. When the training certificate, including the completion date can be located, please send it to OCC for the correction of this non-compliance.
During a review of staff files, it was found that three Aides did not complete basic health and safety training within 90 days of their employment. However, the three of them have since completed Basic Health and Safety training so their portion of this non-compliance is now corrected. Another Aide still has not completed basic health and safety training and it is beyond the 90 days of her start of employment. Please provide the certificate of completion for her Basic Health and Safety course for the correction of this non-compliance.
