There is one child who did not have Part 1 or Part 2 of the health inventory forms in their file and 1 child who did not have Part 2 of the health inventory completed by their health care provider. Provider stated she completed the 2023 basic health and safety update that was due by 3-31-24 and emailed it to LS. Provider could not locate the certificate during inspection today and LS does not have a copy of the certificate printed in the provider's file or by email. Provider should locate the certificate and send it to the LS or complete the 2023 basic health and safety update immediately and send the certificate to the LS. Provider should complete the 2024 BHS update no later than 12-31-24.
Licensing Specialist observed a child size trampoline in the area provider uses for child care, which is accessible to the children. Provider also has a trampoline for her own children that is accessible in front of the house. Child care children are not allowed to use trampolines during child care hours and all trampolines are to be inaccessible to children during child care hours. Please remove the trampoline from the child care outdoor activity area and make the other trampoline inaccessible to child care children or remove it completely. Please send photos and a written plan of correction to LS.
Provider has one Cosco playpen she has been using that has a large hole in the mesh. This is a potential hazard and provider should immediately stop using this playpen. Provider has 4 additional playpens and a crib. Provider stated she will replace the playpen with the hole immediately with another playpen that is not being used. Please send a written correction to Licensing Specialist.
Licensing Specialist observed provider's fire drill and emergency drill record. Provider recorded disaster drills for September 2022 and May, June and October 2023, but did not record fire drills for those months. Provider is reminded fire drills are to be conducted and recorded at least once per month and emergency drills are to be conducted and recorded, in addition to monthly fire drills, at least twice per year. Provider acknowledged understanding and stated she will do this, beginning immediately.
