There are two emergency forms that do not have daily, authorized pick up persons included. Please obtain from parents-guardians and add to children's files. Please send copies to Licensing Specialist.

There is one child enrolled who has an Epi-Pen and Benedryl stored at school. The medications are locked in the nurse's office, but the child care staff have access to the nurse's office. The medication is current and labeled as required, but the medication administration form in the child's file is from 2022. These forms are only good for one year. Please obtain an updated medication administration form or allergy-anaphylaxis action plan form, that is completed by the child's health care provider, from the child's parents-guardians and add to child's file. Please send a copy to the Licensing Specialist.
This mandatory review inspection recorded no violations or advisories.
