One child's Medication Consent Form included a medication dose of 3.75 ML; however, documentation within their Individual Health Care Plan noted a dosage of 1/2 TSP/5 ML. The program must have consistent information to ensure the proper dosage amount will be administered, if applicable.

Current consent for administered medications
A sampling of children's files was reviewed. One child's file did not include a current progress report.
A sampling of staff files was reviewed. 1) One staff member who has been at the program for 6+ months did not have current First Aid Certification. 2) Two staff files did not include current bi-monthly observations.
There were several bottles and sippy cups in the I/T refrigerator that were not labelled. Personal items intended for individual use by children, including but not limited to bottles and sippy cups, must be labeled with the name of the child.
At the time of the visit, the kitchen door was open. There must be a barrier between children's activity space and the kitchen.
At the time of the visit, there was a portable space heater plugged in within the office space. The use of portable heaters and portable radiators is prohibited during child care hours.

Individual Health Care Plan (IHCP) - R Apr 2025
On 11/13/24, the Toddler/Preschool and Infant/Toddler attendance were not accurate at the time of the observation. A child had not been signed out from the Toddler/Preschool Classroom attendance when they went to the Infant/Toddler Classroom. In addition, the Infant/Toddler Classroom did not add the child to their attendance upon arrival. The program must have a method to know which children are present in each classroom at any time in the day.

A sampling of children's files was reviewed. One child's file had a transportation form; however, the parent had not signed/dated the document.
