Toothbrushes were not labeled for each child at the time of the visit. Licensor observed when children were finished brushing their teeth, they place their toothbrushes in the sink with other children's toothbrushes.
Second exit through the garage door is obstructed by a child size rocking chair, ride on toys, and a rocking horse.

Educator is missing the physical address of a meeting place other than her home for emergency situations in the licensed classroom.
Educator has not been using the second exit for evacuation drill. At the time of the visit, she was not able to open the garage glass door due to insulation.
Educator stated she does not use sheets on the mats. Mats were stored touching each other.
Individual towels used for drying hands after handwashing are not labeled for each child.
Licensor observed rough wood exposed on closet door with missing doorknob, Outdoor play structure has a climbing rope to a platform with railings that have large openings that a child could fall through.
