Daily attendance in two classrooms was inaccurate.
Not all opened food packages were stored in air-tight containers.
During the visit, the following was noted: 1. An educator didn't wash her hands after diapering a child. 2. An educator didn't ensure a child's hands were washed after changing their diaper. 3. An educator did not keep one hand on the child when the child was being changed on an elevated surface. 4. An educator did not wash and disinfect the changing table after diapering a child.
In the toddler classroom, a toilet plunger was accessible to children.

The Toddler I Classroom had eight children present, and seven were marked in attendance.
In the Infant Classroom, there wasn’t a thermometer in the refrigerator verifying the temperature.

At the time of the visit the floors in the lobby on the main floor were in disrepair.
There was chipping paint on the front of the building. At the time of the visit the stairwell from the main floor to the preschool playground had chipping and flaking paint on the stair treads. The stairwell leading out to the playground has peeling paint on the ceiling and the wall under the large window has rust and the wall is in disrepair.
The fence has areas around the perimeter that have gaps under them. The preschool playground has a picnic table that has nails sticking out and is accessible to the children. The Tall gate at the side of the back of the playground is in disrepair. There is lattice around an area where riding toys are kept and the lattice has brittle wood and needs to be replaced. Two buckets filled with sand on the playground used for a shade structure need to be more secure. They can be easily tipped over or pulled over. There is a broken little tykes house that is leaning against the fence and is a hazard. Front steps are in disrepair. Large areas of the cement are missing and pose a hazard.
