Diapering and Toileting for children under 2.9
3 staff files were reviewed, and the following was noted: 1. 1 Educator did not have evidence of [REDACTED] 2. Observations and consultations with staff every other month were not on file.
3 Children's files were reviewed and 2 were missing evidence of an updated lead screening.
Flooring was being replaced at the time of the visit, however, 2 classrooms had chunks of linoleum that were missing, and that floor was not replaced.
An Educator in the Infant B classroom took both hands off of a child that was being diapered on an elevated surface.
This investigation inspection recorded no violations or advisories.
At the time of the visit Educators in Infant A and Toddler I did not follow proper diapering/infection control procedures: 1. The children's hands were not washed after changing in Infant A. 2. The Educator's hands were not washed after changing diapers in Toddler I. 3. The diaper changing surface was not washed with soap/water or a cleaner prior to applying the bleach and water solution in both classrooms. 4. The remain wet time for the b/w solution was not followed in Toddler I.
Evacuation procedures and diagram of escape routes were not posted by each means of egress out of classrooms.

When observing in classrooms the following was noted: 1. There we no straps to buckle children in on 2 infant high chairs. 2. The dramatic play sink in Toddler E was missing the plastic sink insert. 3. Caulking around several sinks in classrooms and bathrooms was black with mold/mildew and in need of replacement.

When observing the playground the following was noted: 1. The blue plastic coating and yellow plastic coating on the climber stairs was starting to deteriorate and rusted metal is beginning to show through. 2. There was no sand in the sandboxes on the playground.

When observing n classrooms, the Licensor noted wooden dramatic play kitchen sets were not secured in Preschool M, and Infant E.
1 child's emergency medication was not readily available as it was expired..
Shaving cream was found stored in a low cabinet that was not locked making it accessible to children in the PS M classroom.
