Individual Health Care Plan (IHCP)
A Child's Record did not contain evidence of Medication Consent Forms for three prescribed medications.
A review of three Children's Records at the time of the visit indicated the following: -One child's record contained partial information about the child's diagnosed chronic medical condition, but did not have evidence of a current and complete IHCP (Individual Health Care Plan). The program must maintain an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child's health if the treatment is not administered.
A review of three Children's Records during the visit indicated the following: -A child that has been prescribed three medications did not have a Medication Consent Form for any of the three medications. This means the program does not have parental permission to administer medication to the child in the event of a potential medical emergency.
