Indoor Space
A review of three children’s records indicated that one did not have current evidence of a physical examination.
The following was observed and accessible to children: There was a rusty air vent in the PRESCHOOL 4 bathroom. There was an unsecured and tippable shelf in PRESCHOOL 3. There were two rusty floor air vents in PRESCHOOL 1.
It was observed that the protective covering on the playground climber metal rungs were worn and peeling.
No peeling or flaking paint (indoor and out)
It was observed that there was chipped paint in the CLASSROOM 2 and 3. The program is in compliance with their lead inspection report.
Chipping paint was observed in the following indoor spaces: -On the right side wall at the entrance and in the bathroom below the soap dispense in ROOM 3. -On the wall behind and below the soap dispenser in the bathroom on the left side in ROOM 1. -Along the bottom of the building facing the small ground.

The following was observed in ROOM 3 accessible to children: -Sticky residue on the window pane under the front windows -Tape peeling up from two posters on the right side of the room. -Two rusty brackets in the bathroom that anchor a wooden shelf. -A missing cover where the toilet pipe meets the wall, exposing a hole in the wall. -Rust on the floor vent in the bathroom. The following was observed in ROOM 2 accessible to children: -Rust on the floor vent in the bathroom. The following was observed in ROOM 1 accessible to children: -Rusty floor vents in both bathrooms and in the classroom under the windows facing the playground.
The following were observed outdoors: -Several broken slats on the porch trellis. -unknown vegetation growing on the playground and through the fence. -There were protruding bolts on the fence and several rusty areas. -There was not a sufficient amount of ground surfacing around the play structure on the smaller playground. In addition, there were rusty areas and areas where the metal had separated exposing sharp edges. -There was splintered wood on the "deck" in the small playground. -The trash can was cracked.
A review of 3 records indicated: -1 did not have a completed transportation plan. -1 was not signed or dated by the parent.
The following were observed indoors: -Room 1: There was a stain on the rug and bleach water accessible in a bucket. -Room 2: There was sticky residue on a cabinet, a rusty vent in the bathroom and a bottle that was not labeled. -Room 3: There was debris on a shelf, sticky residue on a cabinet and a vent that had peeling paint.

The following was observed in the outdoor space accessible to children: -A protruding nail from the building and a protruding nail from the wooden stage on the left hand side. -Multiple bolts on the fence and gate connecting the two playgrounds are longer than three threads. -A rusty vent on the outside of the building facing the playground.

There was chipping paint in several rooms and outdoors on the shed and building.
The program was using two different evacuation drill logs. One did not include the "effectiveness" and the other did not include the route of evacuation.
