The following safety hazards were observed and accessible to children: LIVING ROOM: - The changing table was not secured to the wall. - A large water dispenser machine was not secured to the wall, and its cords were accessible to the children. BATHROOM: A box of gloves and wipes was within children's reach.

The children’s records were observed, two children were missing their lead testing.
Provider is not documenting the evacuation drill time on the evacuation logs.
The plastic kitchen in the basement was not secured posing a tipping hazard. In the outdoor space, various toys had sitting water.
Provider has one infant enrolled. The pack and play was observed to have an extra mattress in the playpen. The provider reported the infant has slept on this extra mattress and put it there to make it softer for the child.
