Indoor Space - free of hazards
The living room space has chipping and peeling paint on the window ledge.
There were several electrical outlets in the bedroom which were not covered.
The bedroom was not set up for activity space. The bedroom will need to be set up for activity space to be included in the square footage for outside of napping purposes.
The front door outside door was broken, the latch was broken and the screen. A kitchen drawer had gloves and plastic bags accessible. In the bedroom, a piece of furniture had sharp edges not protected. The oven knobs are not covered and in reach of the children.
All of the children's progress reports were reviewed. Four children did not have completed progress reports on file.
On the day of the visit, the educator and their assistant were present with nine children. Three children were under the age of two years old. The educator has four infants enrolled. Per regulation 7.10(04)(g) the educator is not allowed to have more than three infants enrolled with a group size of seven to ten children.

The educator did not have a complete file for the assistant. The file was missing the assistant's trainings, CPR/First Aid and medical form.

In the bathroom, Clorox disinfectant wipes were accessible to the children. In the kitchen, one drawer had plastic bags accessible.
The educator did not have a complete staff record for their assistant. The staff record was missing the staff’s medical form, CPR/First Aid and trainings completed. The educator is not documenting the staff attendance.
The first aid kit was missing several of the children's emergency cards, various of the emergency cards were expired and an ice pack was not included.

The light panel in the bathroom was uncovered exposing the wiring in the wall and the shower edge is stone and did not have a safety barricade on the edge.
Two rain gutters attached to the home were broken and there was spray insulation was exposed on the side of the outdoor wall.
In reviewing the children's records, two children required lead testing, one child’s general permission was not fully completed as well as his emergency card.
The first aid kit was missing the children’s emergency medical cards.

All of the children's records were reviewed. Four children did not have an annual update form completed, two children did not have their lead testing, one child did not have documentation of their physical or vaccines. Three children have expired physicals on file.
The educator has not conducted an evacuation drill since September 2023.
The educator was unable to provide documentation of completed ongoing professional development for the required hours.
Five of the ten enrolled children did not have completed or updated progress reports.
There are two children with a documented food allergy with no required posting.
The outside path of the second exit was obstructed by a infant jumper and an RV parked in the driveway.
The parent handbook was not fully completed and provider has not been providing it to the parent's of the children enrolled.
