Six of the ten enrolled children's records were reviewed. Of the six reviewed, two children require their vaccination list, two children did not have their annual update form completed, four of the children's emergency authorizations were not fully and or correctly completed.
The provider is not documenting the evacuation length of the drill.

Ther is an appropriate amount of indoor materials. Educator reported the the outdoor materials were still at her assistants home. Educator will need to provide pictures of materials.

Water Temperature
The water is too hot for the children.
There were knives, a cheese grater, and cords accessible to children in the kitchen. There were cleaning and hygienic products accessible to children in the bathroom. Also the trash can in the bathroom didn’t have a lid.
The outlets in the kitchen and dinning room were not covered. The baseboard heater in the playroom didn’t have a cover. The outdoor space was safe.
The first aid kit is missing a roll of gauze, scissors and a thermometer.
The basement closet where the electrical box is stored was accessible. Five shelves in the basement level were not bolted and could pose a tipping hazard. The toy kitchen wasn't bolted and could pose a tipping hazard. The television cord in the basement was accessible. The trash wasn't covered. There was no gate across the basement stairs making the stairs accessible.

The second exit is via the deck. There is a drop from one side that will need a 4 foot barricade. The front exit is missing an exit sign.

One climbing structure measures more than 18 inches. There is no ground surfacing.
There is a slope that divides the yard. Four foot fencing is required. The trampoline can’t be used with the children.. Lastly, two A/C units are accessible and a long piece of broken wood is accessible.
In the kitchen, a cabinet was unlocked allowing access to medication. The cabinet under the sink was unlocked allowing access to cleaning material and roach killing bait. The stove knobs were not covered and in reach of the children.
