Reviewed children’s files. There were multiple files that had missing information on the enrollment packets. There was one file that did not have lead screening results. There were two children present that did not have completed enrollment packet and physicals on file.

Educator has been out of her ratios, based on attendance this occurred between March and April 2026. Educator has had 7-8 children present at one time, none are school age children. It was reviewed with the educator that preschoolers are not considered school age, they must be 5 years old and older. Educator will need to readjust her schedule and submit copy of daily schedule with children present and ages.

Attendance records are not accurate. Educator is not adding her own child, who does count into her capacity. Attendance for today did not reflect the names of all of the children present.
It was observed that there were multiple items on top of the diapering mat.
It was observed that the two pack and plays did not have individual fitted sheets.
It was observed that the cover for one of the heater baseboards was loose and coming off.
It was observed that toy shelf was easily moveable. There was access to baby wipes under the diapering table.
It was observed that children had free play throughout the visit; there was no observations made of more structured activities and transitions were not observed to be clear for the children.
This change of space visit inspection recorded no violations or advisories.

It was observed that the stairs leading to the yard from porch did not have a hand-railing on one side of the stairway.

Attendance for 6/27 and 6/30 was not documented.
It was observed that there was large gaps between the fence and the ground leaving sufficient space for a child to go under the fence. There was an above ground pool observed, the pool measures under 4 feet tall and did not have a barrier installed to prevent access to the pool.
This follow-up visit inspection recorded no violations or advisories.
Children are due for updated progress reports.
It was observed that there was some construction work being done on the educator’s porch, which is the educator’s second. emergency exit. There was access to multiple hazards, such as a large saw, working tools, paint cans, nails, wooden boards, extension cords, there was also a large opening and drop, which measured 33 inches from top to bottom; next to the exit door that needs to be secured. There was access to multiple wooden boards and some broken glass on the side of the deck that the educator needs to prevent access to. It was reviewed with the FCC supervisor and educator will need to prevent access to all hazards and close the large opening. Educator must submit proof of such by today, 3/4/25 in order to be able to open program tomorrow 3/5/25, otherwise program will need to close until educator submits proof. Se observó que se estaban realizando algunas obras de trabajo en el porche de la educadora, que es la segunda salida de emergencia. Había acceso a múltiples peligros, como una sierra grande, herramientas de trabajo, latas de pintura, clavos, tablas de madera, cables de extensión, también había una gran abertura/ caída, que medía 33 pulgadas de arriba a abajo; junto a la puerta de salida que debe asegurarse. Había acceso a varias tablas de madera y algunos vidrios rotos en el deck, la educadora debe evitar el acceso. Se revisó con la supervisora y la educadora deberá evitar el acceso a todos los peligros y cerrar la gran abertura. La educadora debe presentar prueba de ello antes del fin del día hoy, 3/4/25 para poder abrir el programa mañana 3/5/25, de lo contrario, el programa deberá cerrarse hasta que la educadora presente la prueba.

The gate installed at the top of the stairs is no longe an approved gate. This mesh gate must be replaced with a permanent secured gate. El gate instalado en el segundo piso ara prevenir acceso a las escaleras no es uno que se puede aprobar. Se debe remplazar con un gate permanente y asegurado que no sea de tela.

Reviewed children’s records. It was observed that there were three files in which children did not have lead screening indicated on their physicals. One file had missing parent signatures and date of signature on page 3, 5, and 7 of the enrollment packet. Page 7 needs to be completed by parent without leaving any blanks. It was observed at time of visit that there was a 7-month-old infant sleeping in a bouncy chair with a blanket. Educator stated that is how infant sleeps. Licensor reviewed safe sleep regulations, policy, and practices. Licensor requested that educator move the infant into a pack and play and remove the blanket. Educator moved the infant to a pack and play. En el momento de la visita se observó que había un bebé de 7 meses durmiendo en una silla mecedora con una cobija. La educadora afirmó que así es como duerme el bebé. La licenciadora revisó las regulaciones, póliza y prácticas de sueño seguro. La licenciadora solicitó que la educadora pusiera al bebé en el corral y removiera la cobija. La educadora trasladó al bebé al corral.

Educator has not been keeping daily attendance for children to reflect who is present. Discussed having attendance records reflecting actual time of arrival and departure. La educadora no tiene record de asistencia diaria de los niños que refleje hora de llegada y salida actual.
Children’s files were reviewed. One file had an incomplete general permission. The two other children enrolled did not have enrollment packets, one had no physical, and other needs individualized health care plan for medical condition.
It was observed that children did not wash their hands prior to sitting down to eat their lunch. Se observo que los niños no se lavaron las manos antes de comer.
At time of visit there was some free play observed. Educator was observed guiding the two older children to some developmentally appropriate play activities and engaged with the two children. It was observed that the television was on the entire time although children were not actually watching. Discussed limiting time television is on.
