1 child with an IHCP for [REDACTED] did not have the required medication per the plan on site.
When the Licensor entered the Multi-Age B classroom 1 Educator was alone with 13 children, the second Educator arrived after approximately one minute.
When observing in classrooms the following was noted: 1. Thermometers were missing from classroom refrigerators. 2. Edges to a multi-colored foam climbing mat were torn and not impervious to bodily fluids in the Toddler A classroom.

When observing in the Toddler B clasroom the following was noted: 1. The child and the Educators hands were not washed after the Educator changed the child's diaper. 2. The Educator did not remove her gloves after changing the child's diaper and cleaning the diaper changing surface. She went around room picking up toys, touching the IPAD, attendance sheet, water jugs, snack items, washing down counter and sweeping floors. 3. The paper towels were not in a holder the Educator was touching the role with dirty hands and using them to clean the counters and table. 4. The diaper changing surface was only sprayed with the glass multi-surface cleaner and not disinfected after cleaning. 4. The tables were only sprayed with the glass multi-surface cleaner and not sanitized. 5. The Infant A sink was being used to warm a bottle, this is the same sink that they use for handwashing after diapering. They must use a different sink for all food prep.

3 children's files were reviewed, and the following was noted: 1. 1 child's file was missing the date of admission, the transportation plan was not signed/dated by the parent, the developmental history was not dated by the parent and this child had a chronic medical condition ([REDACTED]and did not have an IHCP in the child's file. 2. 1 child did not have evidence of [REDACTED]
3 Staff files were reviewed, and the following was noted: 1. 1 staff was missing evidence of an updated [REDACTED]
When observing in the Toddler B classroom the following was noted: 1. An Educator was not appropriately supervising the children in her care. The educator was cleaning counters and tables, sweeping the floor, washing sippy cups in the sink (with her back to the group) while the other Educator was trying to gather children to do circle. 2 ABA therapists arrived, and the Educator continued to clean leaving the other Educator to supervise the group and speak to the ABA therapists. 2. The Educator in circle with the group removed a foil leaf from a child's mouth and placed it next to him on the floor, another child crawled over to the leaf and picked it up and put it in his mouth, the Licensor had to tell the Educator.
The Infant Room fridge temperature was 56 degrees, the temperature needs to be between 32-40 for safe food storage. It is recommended the thermometer be kept towards the back of the fridge for a more accurate reading.
1 Child's medication consent form was not accurate, it stated under dosage "before nap" it did not state the dosage of the actual medication. The form was not filled out completely as required, it was missing the reason, the time, the start date/stop date, whether it needed to be refrigerated and what route it is to be given.
1 child's IHCP was not complete, the possible side effect of treatment was not listed. The plan stated to administer [REDACTED[ , the program was administering 1 puff daily before nap time every day.

At the time of the visit the following was noted: 1. Folding metal chairs were blocking the 2nd means of egress/emergency evacuation route for the Toddler A classroom. 2. There was no paper towels in the PS A and Toddler B children's bathrooms. 3. A toilet bowl plunger was found lying on the children's bathroom floor in the PS B classroom.
6 staff files were randomly reviewed, and the following was noted: 1. There was evidence of a self-evaluation however; there was no documentation of an annual program evaluation for any staff members.
Well-balanced curriculum that supports the health & well being of children
When the Licensor arrived an Educator from the Toddler A classroom was standing in the doorway to the PS B classroom, the Educator left the Toddler B classroom out of ratio; the teacher in the Toddler B classroom was alone with 8 children.
8 children's files were reviewed, and the following was noted: 1. 3 files were missing evidence of an updated lead screening.
When observing in classrooms the following was noted: 1. In the PS B classroom all 18 children were seated doing an art project: gluing cotton balls onto ghosts. The Licensor heard a child ask for more glue and the Educator responded "sorry friends, I don't have anymore, I had to share it with another classroom, I'll get some more tomorrow." There must always be a sufficient quantity of materials. 2. there was not a sufficient amount of toys/equipment accessible to children in the Toddler B classroom; more toys/shelving units need to be added for independent access.

When observing the playground the following was noted: 1. The cover to a toy box was broken off and lying up against the fence. 2. The doors to the shed are starting to rot, rusty nails, broken wood and holes were exposed.
Written emergency evacuation plans were not posted by the door leading outside in the Toddler B/C classroom.
1 child with a diagnosed medical condition on their most recent physical exam was not listed on the allergy, chronic medical condition/emergency medication list.
1 child did not have an updated IHCP on file.
When reviewing medication the following was noted: 1. 1 child's prescription for an inhaler stated to discard on 8/4/2023.

In the Toddler C classroom, an educator did not probably disinfect the diapering surface between children's diaper changes.
In the Toddler C classroom, an educator did not wash their hands or the children's hands after diapering.
Diapering
During a diaper change in the Infant 1A classroom, it was observed that an educator didn't keep at least one hand on the child when opening the drawer to grab the diaper cream.
Indoor Space
When observing in classrooms the following was noted: 1. Caulking behind/around the large sink in the Toddler B classroom was starting to mold/mildew and is in need of replacement. 2. There was no gate in the Toddler B classroom to prevent the children from entering a storage area that was filled with stacked furniture posing a potential hazard. 3. The Toddler B refrigerator did not contain a thermometer.
