During the Health and Safety review of the program, the following was found: 1) The diapering/toileting plan was not posted in the diapering area (Changing table) in the Toddler Classroom. 2) The diapering/toileting plan was not posted in the bathroom of the Preschool Classroom.
The program must ensure that food/snacks are not served to children beyond their recommended dates of use. During the inspection of the Program's Kitchen, clear containers filled with children's snacks were found without my dates or expiration dates.

The playground did not have an adequate depth of an impact absorbing material (mulch).
During the visit, the following was found: 1) An Educator in the Infant one Classroom did not wash her hands before handling food. 2) Children in the Toddler Classroom did not wash their hands after eating. 3) An educator in the Toddler Classroom did not maintain a sanitary environment by not ensuring that the tables after snack time were washed and disinfected properly. The educator only used soap and water to clean the tables.
At the time of the visit, the program did not maintain accurate attendance: 1) In the Infant/Toddler Classroom, three children were present, but only two were signed in. 2) In the Toddler/Preschool Classroom, five children were present, but six were signed in.

At the time of the visit, neither the building nor the fire inspection was current.
During the safety inspection of the program, the following was found: 1) In the Toddler/ Preschool Classroom, phone charger, radio, and air purifier cords were found hanging down from the outlets in children's reach. 2) The blue mats in the Infant/Toddler were found ripped and the foam exposed.

Four staff files were selected to review, and the following was found: 1) One staff member One staff member did not have evidence of a current [REDACTED]2) One staff member did not have evidence of [REDACTED]. 3) Three staff members did not have evidence of bi-monthly observations.

During the safety Inspection of the outdoors/playground, the following was found: chipping and peeling paint were found on the white panels of the building on the Toddler/ Preschool playground.
Five staff records were reviewed, and the following was found: 1) Three staff members did not have evidence of a current Physical. 2) Three staff members did not have evidence of [REDACTED] 3) One staff member did not have evidence of a First Aid and CPR certificate. 4) Two staff members did not have evidence of a completed EEC Essentials Certificate.

Diapering and Toileting for children under 2.9
Eight children's records were randomly selected for review, and the following was learned: 1) Two children did not have evidence of date of admission. 2) Two Children did not have evidence of a current physical.
During the safety inspection of the program, the following was found: 1) Multiple Shelves in the Toddler/Preschool Classroom were not secured. 2) Two Rugs in the Toddler/Preschool Classroom were rolling up and causing a tripping hazard.
During the safety inspection of the program Toddler/Preschool playground, the following was found: 1) The screen on the fence was secured down to the fence. 2) Trash was found in multiple areas on the Toddler/Preschool playground.
While on the playground, an Educator from the Toddler/Preschool Classroom wiped a child's nose and did not wash their hands afterward.
During a diaper change, the following was observed: 1) An Educator in the Infant Classroom did not keep at least one hand on the child when the child was being changed on an elevated surface. 2) A child's hands were not washed with liquid soap and water, and dried with individual or disposable towels after a diaper change in the Infant Classroom.

A review of one medication consent showed it was missing the parent's signature.
Educators were observed yelling across the playground at preschool children rather than speaking to them in a calm tone at their level, explaining the expectations.
Equipment is clean, safe, secured, and poses no hazard.

An educator was observed looking her phone on the playground when she should have been supervising toddlers.
Furniture throughout the program was not secured.
The following was observed: 1. The Preschool classroom floors, rugs, and bathrooms were very dirty. 2. A corner of a rug was curled up causing a tripping hazard.
The playground's fall zones did not have the required 9-inch depth.
The Infant/Toddler classroom did not have allergies posted. Through interviews with staff, it was learned there was a child enrolled in that class with a dairy allergy.
A review of one IHCP showed it was missing the required training information.

At the time of the visit, the following was found: 1) An educator was observed in the Toddler classroom with five toddlers, while the other educator was in the two's classroom with one toddler, leaving the educator out of the ratio. 2) The attendance in the Two's classroom was not accurate. There were six children signed in, and seven were present.
During the safety inspection of the playgrounds following was found: 1) On the Toddler playground peeling and chipping paint was observed on the building. 2) In the Playground bathroom rust and chipping/peeling paint was observed on the wall vent.

Three children's records were randomly selected for review, and the following was learned: 1) 2 children did not have evidence of a current physical exam. 2) 3 children did not have evidence of a current progress report.
Three staff records were randomly selected for review, and the following was learned: 1) Two Staff needed evidence of a staff form. 2) Three Staff needed evidence of documentation of the interview. 3) Two Staff needed verification of two references. 4) Two Staff did not have evidence of the Date of Hire. 5) Two Staff needed evidence of current physicals and two doses of[REDACTED] as required by regulation. 6) One Staff needed a completion Certificate for the EEC Essentials Training. 7) Three Staff needed evidence of completion of the Programs Orientations.
During the safety inspection, the following was found: 1) Toddler, Two's, and Prekindergarten Room 1 classroom, multiple shelves, and dramatic play equipment needed to be secured. 2) The diapering pad in the Toddler room was worn, and the inside foam was exposed.
During the review of the playground, it was observed that the program did not have adequate depth of an impact-absorbing material (Mulch).

When observing in classrooms the following was noted: Sunscreen diaper cream and hand lotions were found accessible to children in the Blue room (2's) and Purple Room (PS3).

1 wooden shelving unit was not secured and was tippable in the Yellow Rm (Toddler 1) and Orange Rm (PS 1 downstairs)

Written emergency evacuation procedures were not posted by each exit as required. Diagrams of escape routes were posted.
The diaper changing table was not washed with soap and water prior to disinfecting in the Yellow room (Toddler 1).
When reviewing IHCP's the Licensor noted 1 child's IHCP expired and 1 child with a chronic medical condition (febrile seizures) did not have an IHCP on file.
1 child's IHCP stated to administer Ibuprofen however the parent provided the program with [REDACTED]
