The certified assistant has not provided the educator with physical or virtual copies of her professional development training certificates.
Educator has not been documenting the assistant's attendance and stated that she did not know that she had to document it.
The toothbrush holder appears to be stained with toothpaste residue and dust.
Napping Room: The Licensor observed clutter caused by pile of clothes stored on the bed and overflowing from the basket and toys stored on top of others in an unsafe manner in the napping room. clutter caused by excessive toys stored on top of the other in an unsafe manner in the playroom. There are also clothes on the chair and floor, there are bags of unknown items and a mat stored on top of the bookshelf in an unsafe manner. Playroom: there are multiple/extra toys stacked one on top of the other in an unsafe and unsecure manner.
The first aid kit is missing the medication for the child with a medical condition.

Bathroom: the vanity contains cleaning products and beauty products in it and the safety lock on the door is in disrepair.

A pack n play and another mat were not available to review at the visit. Educator does not have enough mats for 8 children.
Educator was observed only interacting with the children when they needed to be redirected or stopped from doing something that could harm them.
Educator was observed bringing the children into the master bedroom, which is not licensed space.
Children were observed running around the home, yelling and arguing with each other. One child was climbing on the table and into the shelves.
Educator does not have ample or age appropriate materials for the children she has enrolled. The materials that educator has for older children are dirty.
Educator is missing annual updates for all children.

The educator uses ASQ for progress reports. However, she is not completing them consistently.
There is broken glass on the floor within the children’s reach outside of the kitchen exit route.
