Several children's coats, hats, etc, were observed on the floor. The program must provide sufficient space, accessible to children, for each child to store clothing and other personal items in a safe and sanitary manner.
Individual Health Care Plan (IHCP)
One child's Individual Health Care Plan included a second dose of medication if needed. The parent has only provided the program with one dose of the medication.

In the cafeteria, there was a large plastic container of disinfectant wipes that were accessible to the children.
Toxic Substances
A sampling of staff files was reviewed. 1) Two staff files were missing evidence of the current PQ Registry. 2) One staff file was missing evidence of a current physical exam. 3) Two staff files did not include at least 20 hours of professional development for the previous year. 4) One staff file did not include a current annual evaluation.

In the cafeteria, two exits did not include a written evacuation plan and escape diagram.
At the cafeteria observation, the kitchen door was open. There must be a barrier between the children's activity space and the kitchen.
Emergency medications readily available
In the gym, there were 2 doors that lead directly to the outside that did not include written evacuation plans that included where to meet once outside in an evacuation.
One child's Individual Health Care (IHC) plan included a medication and dosage for a minor allergic reaction. The medication consent form included a different medication and dosage for a minor allergic reaction. The doctor signed both documents. Please ensure that medication and dosage amount is consistent.
The program did not have a method of knowing who was present in Room 16 at the time of the visit.

Physical Space Inspections
1) One child's Individual Health Care Plan (IHC) included dosage of 3-4 tablets; another current IHC Plan included dosage of 4 tablets. The Medication Consent form included dosage of 1-2 tablets. Parent and doctor authorizations must include the same information. In addition, the dosage amount cannot be a range. 2) One child's IHC plan included a second dose if needed. The program only had one dose of the medication provided by the parent.

One child's Individual Health Care Form included an emergency medication for a mild reaction. The parent has not provided this medication along with the Medication Consent form to the program.
There were several bottles of cleaning supplies stored accessible to children.
