One child with a [REDACTED] Parent has not signed the EEC Medication Consent form. This form must be signed and placed in the child's file or with the emergency action plan that the doctor signed.
Fenced in outdoor play area in the back of the home surrounded by a chain link fence. Little Tykes climber in a sand enclosure. Observed weeds in the sand area and sand must be replenished before climber is used with the children.

If windows are ever opened, they are opened from the top as the program is located on the second floor of the home. Screen door leading to the deck and outdoor play space had a large rip in the metal screen.

Indoor Space - free of hazards
The dining room on the first floor that the educator wishes to add to the license has a fireplace hearth that is not padded and a wood stove that either needs to be padded or gated to make inaccessible to the children. Educator stated the stove is never used. The kitchen next to the dining room has a gate in the doorway to prevent access to that room.Educator must send licensor photographs of the corrective actions when completed.
