A review of four staff files showed the following: * One staff file showed a BRC that has been expired since January 2023, * One staff member's First Aid certification was expired * Three of the four staff files were missing a completed orientation, application or resume, first aid/CPR certification, essentials training, evidence of interview and two references, evidence of a physical and [REDACTED] EEC Essentials trainings.

A review of staff BRC's showed one employee's BRC expired January 14, 2023.
The program did not follow their BRC policies when they allowed an educator to continue working with an expired BRC for over a year.
During a review of medications the following was observed: *One student's seizure medication did not include the prescription label and did not have a current consent from the physician. *One medication consent was not signed by the parents.
A review of six IHCPs showed one was expired, three were not signed by the parent, three were missing the required training info, and one was missing side effects of treatment and potential consequences if treatment is not provided.
During a review of emergency medications, it was learned one student's required Benadryl was not stored in a readily accessible location.
