Exit signs are not posted as required. Plan for meeting potential emergencies is located in the parent handbook.
Only two children have EEC progress reports that were completed in October 2025. All of the children enrolled are due to have an evaluation in their files. Written progress reports must be completed every three months for infants and children with special needs, every six months for toddlers and preschoolers, and once a year for school age children (5 years and older).

First Aid Kit (FCC)
Random review of children's files. Most enrollment packets were current or annual update forms signed in November 2025. One child has an outdated enrollment packet and requires the parent sign an annual update form. The same child is missing current physical and immunization documentation.
In the first aid kit, one child needs an updated emergency card page. These pages must be updated every year.
Refrigerator temperature was 44 degrees. Temperature must always be between 32 and 40 degrees.
Hot water temperature to the bathroom sink was tested at 135 degrees F. Hot water to the sink where the children wash their hands must not be hotter than 120 degrees F. Discussed the educator shutting off the hot water to that sink.
Sandbox has a tarp but was observed to not be covered properly at the time of the visit. Standing water was present on the tarp.
An emergency contact phone list was not posted as required.

Attendance Logs
Two wooden toy shelves are not secured to the wall in the living room and pose tipping hazards.
The educator stated that she renewed her CPR and First aid last week. Educator must email the licensor written proof of course completion.
The educator is tracking attendance for the children on the computer for the food program. At the time of the visit the educator's internet was down so attendance was not accessible. Discussed the educator also keeping a written attendance that is available at all times.

Indoors- Two tall wooden shelves are not secured to the wall in the living room and pose tipping hazards. Outdoors- A gate is needed for the bottom of a set of stairs on the side of the garage.
The educator has not conducted a fire drill with the children since reopening her program in October. Evacuation drills must be conducted and logged each month.
The educator is only tracking attendance for the children on the computer for the food program. A written attendance that is accessible to others must include the daily arrival and departure times of the children recorded.

The back yard is in close proximity to a large body of water. There is a section on the back and by the deck that the barricade is not four feet high. Also, multiple sections of the wooden fence enclosing the play area are not four feet high. ( 44, 46 inches high).

The educator has been using the back yard with the child care children. The back yard was not approved for use because the educator lives in a busy street and the fencing is not adequate to prevent access to the street.
