Attendance Logs
Attendance is recorded on a sign in sheet with the daily arrival and departure times of the children. Educator asks the parents to fill in arrival times and educator fills in departures. On the day of the visit three children were not signed in. Attendance must always be accurate with times filled in as they occur. Discussed the educator filling in the attendance for the parents.
Water Temperature
Hot water temperature in the bathroom sink was tested at 125 degrees. Hot water where the children wash their hands must not exceed 120 degrees.

This monitoring visit inspection recorded no violations or advisories.
One child with multiple allergies, including nuts, with an [REDACTED]. The child has a medical action plan in her file, but plan was last signed by the doctor in September 2021. This form must be updated annually by the doctor. According to the educator, the child has a doctor's appointment this Friday. Parent has not signed the medication consent form for the EpiPen. This form must be signed and also kept in the child's file.

[REDACTED] etc. Allergy list was reviewed but is not posted and does not include the location of the child's [REDACTED]. Location of EpiPen must be posted on the door of the closet as well. [REDACTED] is in the prescription box, however, was located in a locked metal box. Discussed the medication being readily accessible to the educator in the event of an emergency. EpiPen must go outside with the child. Discussed placing the EpiPen in the mobile first aid kit.

Observed numerous electrical outlets not covered in the naproom. Observed an outlet uncovered in the family room.
First Aid Kit (FCC)
Observed a treadmill and a standing fan plugged in in the naproom. Discussed removing boxes and other various gym equipment out of the space. Large white shelving unit, shelf with plastic bins, and a colorful shelf in the lower level playroom are not secured to the wall. Large flat screen television in the family room is not secured to the wall or furniture.
Two first aid kits- one for the first floor and one on the lower level. Emergency card pages for the children were out of date. These pages must be updated annually by the parents.
