The program did not have the written parental medication consent form for a child that has a [redacted] allergy and may require medication. The program had the required medication for the child with the [redacted] allergy; however, the medication was expired. The program must gather new medication as soon as possible. The program did have the required individual health care plan.

At this visit, the licensor observed a body of water next to the area that is used for outdoor play. The outdoor space is going to require a permanent barrier to prevent the children from having access to the stream. Please indicate what steps will take place when supervising the children during outdoor play and your plan for adding the permanent barrier. The front porch has chipping paint. The program must be free of chipping and peeling paint. Please indicate your plan for correcting the peeling paint. The water temperature was tested and reading at 115 degrees F.
The side exit was covered with ice. Exits used as escape routes must be kept accessible and free of hazards.
