Children were observed swinging on the swings in the outdoor space. The Licensor measured the depth of the wood chips in the fall zones under and around the swings and the depth was approximately 5 inches. Progress reports were not conducted once per year for school age children.
During a review of staff records, it was found that staff orientation checklists were missing from all educators' files and health records were missing from two educators' files.
The depth of the wood chips does not meet the 9 inch requirement in the fall zones around and under the swings and climbing structures in the outdoor space.

The staff schedule was not current and it was not posted.
The program did not maintain accurate attendance when the program administrator was recording attendance on the Ipad and groups of children were outside and in the gym.
The fire inspection is expired.
One slide had a large crack and the caution tape at the top of the slide was ripped and children had access to the slide.
There is no record of evacuation/fire drills.
Educators were not knowledgeable about children's chronic medical conditions and Individual Health Care Plans.
The medication consent form was missing the parent's signature.
An Epipen was not readily available for a child with allergies when the children were in the gym and in the outdoor space.
The impact absorbing material under and around the swings and climbing structure did not meet the 9 inch requirement. Licensor measured 4 inches in the area around the slide and 5 inches under the swings.
This investigation inspection recorded no violations or advisories.
Evacuation drills were not conducted every month. On the fire drill log, two drills had been conducted from September to April.
The depth of the impact absorbing material was measured at 6 inches under and around the climber and 4 inches under the swings.
