1) At the time of the visit and after checking the crib’s manufacturing label, the Licensor was unable to determine if the crib the program uses for evacuation meets the standard of an evacuation crib. Please submit proper documentation for the evacuation crib and corrective action plan. 2) The evacuation crib mattress has signs of wear and tear.

The space was still under construction. Parts of the floor were missing, trim was missing and allowed access to inside of walls, shelves in the toddler room had rough edges. There was construction materials, paint and cleaning supplies within reach.

It was observed that the outside of garbage cans were dirty. Some of the rugs were also dirty.
