Supervision of Children
On 9/23/25 at 10:00 AM an educator left one preschool child on the playground alone for approximately 30 seconds. The educator closed the door and realized they had left a child outside. Educators must exercise appropriate supervision of the children in their care at all times.

A review of the staff meetings held in the last year indicated that none of the meetings had documented the length of the meetings.

In Toddler C, there were ten children marked as present on the attendance list however, there were nine children present.
This investigation inspection recorded no violations or advisories.
Indoor Space
Documentation of medication administration; Current consent for medications
The following was observed and accessible to children: INFANT B- In two of seven cribs, there was what appeared to be food debris on the mattress sheets. Staff stated that children do not eat in the cribs. TODDLER B- On the door leading outdoors there was rust.
A review of five children's files indicated two medication consent forms were incomplete.

The following were observed: -A small amount of chipping paint. -gloves were accessible in the bathroom.
