In the Preschool classroom, a wooden bookshelf was not secure to prevent tipping.
On the playground, there was an inadequate depth of impact absorbing material under and around the climbing structures.
One child's Individual Health Care Plan has expired. One child's Individual Health Care Plan did not include the physician's signature as required.

In the Toddler Classroom, one electrical outlet was missing a safety cover to prevent children from accessing it. This was corrected during the visit.
The following hazards were observed on the playground. Bolts on the chain-link fence were protruding inward towards the playground in two areas. These areas were shown to the director during the visit. The yellow climbing wall did not have an adequate depth of impact absorbing material under and around the fall zones. The bike merry-go-round did not have the required six feet fall zone near the pavement (the fall zone was approximately four feet).
