The following was observed and accessible to children: In TODDLER A, there was a large crack on the bottom of a toy shelf. In PRESCHOOL 2A, there were two cracked floor tiles.

Emergency Evacuation
In TODDLER A, it was observed that the refrigerator temperature was inconsistent. Initially, the thermometer read 20°F. It was moved by the Program Director, and approximately 30 minutes later, it was rechecked and read 48°F. The licensee must provide refrigeration and storage for food at not less than 32°F, nor more than 40°F, for all food requiring refrigeration

A review of the evacuation drill log indicated that there was no documentation of drills completed in July and August 2025. The Program Director stated that they were conducted, but not documented.

A review of three staff records indicated that a new hire did not have evidence of EEC Essentials 2.0 trainings completed.
The following was observed and accessible to children: The protective shelf covers were ripped in INFANT B.
A review of the evacuation drill log indicated that the effectiveness of each drill was not documented.
A review of six medications indicated that one was expired, and one was not stored with their original manufacturer's packaging.
It was observed that there was a Magic Eraser sponge located next to the handwashing sink.
The following was observed and accessible to children: In the PREPPER A classroom, there was a broom and dustpan located next to the garbage can. In the PRESCHOOL 1A classroom, the play kitchen was unsecured and tippable and there was a broom and dustpan located next to the sink.
It was observed that in the TWADDLER A classroom, there was a hand spray and room spray located next to the handwashing sink, accessible to children. The manufacturer's warning stated to "keep out of reach of children."

The staff schedule posted was not current.
It was observed that in the TODDLER B classroom, the refrigerator thermometer read 42 degrees. This is over the allowed range of 32 to 40 degrees for food storage.
It was observed that in the PRESCHOOL 1A classroom, there were two children's cups without their names labeled.

In the kitchen, there was an open package of crackers not stored in a covered container. The crackers were discarded during the visit.
