Individual Health Care Plan (IHCP)
Five child records were reviewed and found: One child's CPR/First Aid consent form was not signed and dated; One child's medication consent forms for two medications, the Asthma Action Plan from the doctor, and IHCP for asthma were missing; One child's CPR/First Aid consent form was missing; and one child's IHCP for allergy, two medication consent forms, and CPR/First Aid consent form were missing.
Individual Health Care Plans were reviewed and found all of them were incomplete. Each IHCP was missing the names of educators trained to administer medication.

Evacuation procedures posted at each exit
Two staff records were reviewed and found: one staff's Staff Information Form was incomplete (only had name printed at the top) and the orientation checklist incomplete; and another staff's Staff Information Form incomplete and their orientation checklist was missing.
The following was observed during the visit: one staff member supervised 20 children alone transition from the school bus into the program, up the stairs, and to the classroom. Ratios: The program must have the number of educators necessary to: ensure adequate supervision of the group at all times.
The evacuation procedures were missing from the Teen Center exit door towards the Wellness Center.
A review of the evacuation drill log indicated that a drill was not documented for April.

During the visit, staff could not accurately report the number of children they were assigned to supervise. In addition, attendance was not reflective of the children that each group was assigned to supervise once they separated into individual groups.
There was chipping paint on the walls in one room.
