At the time of the visit, in a review of two IHCPs and the corresponding medication consent documentation, the documentation was not fully complete. One student was prescribed two medications and was missing a medication consent document for one of their prescribed medications. All medications must be accompanied by written parental consent and healthcare practitioner authorization when required.

At the time of the visit, in a review of three educators files, the documentation was not fully complete. Two educators were missing documentation of annual evaluations. One educator was missing documentation of date of interview. The program must maintain a personnel record for each staff member as outlined by the staff record checklist.
At the time of the visit, it was observed that an educator who was not teacher certified was alone in ratio with eight preschool students. During the visit, the Director joined the uncertified educator in the classroom. It was observed that the uncertified educator assisted in taking a student to the bathroom, that was located outside of the classroom and downstairs. For purposes of calculating staff to child ratios, staff shall include only those EEC-qualified persons who are assigned to care for children for specific hours of the day.
At the time of the visit it was observed that the gate blocking the stairwell in the Preschool classroom was broken and no longer latched closed. The licensee must ensure that the physical facilities are safe, clean, in good repair and free from hazards and clutter. The licensee must monitor the environment daily to identify and remove or repair any hazards that may cause injury to children.
At the time of the visit in a review of two students IHCPs the documentation was not fully complete. Both students were missing documentation of the educators trained on their health condition. One student was missing documentation of the potential side effects of their treatment and potential consequences if medication was not provided. The licensee must maintain as part of a child's record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child's health if the treatment is not administered.
At the time of the visit it was observed that the infant/toddler classroom had a diaper pad that was made of a foam material and covered in cloth. In programs serving children who are under two years and nine months of age and/or not toilet trained, the educator must ensure that: the changing surface is smooth, intact, impervious to water and easily cleaned.

A review of five staff records indicated the following: -Three did not have evidence of a completed program orientation checklist. -Three did not have evidence of a completed EEC essential training certificate.
A review of four children’s records indicated that one record did not have evidence of a current list of emergency contacts.
It was observed that a Swiffer duster and a staff’s purse were accessible to children at the bottom of the stairs.
This capacity increase visit inspection recorded no violations or advisories.

Five staff records did not have evidence of a completed background record check.
It was observed that there were seven children present in the Infant/Toddler classroom and six signed in on the attendance app. This was immediately corrected by the Program Director.
Documentation of staff meetings were not maintained. A staff meeting is scheduled later this month and will be documented to include the date, time and topics discussed.
The staff checklist was not fully completed.
A review of all children's records indicated the following: -seven did not have a parent signature or date in the enrollment packet. -six did not have evidence of a current physical examination. -one did not have the parent's information listed in the enrollment packet.
There were electrical cords unsecured and accessible to children in both the downstairs and upstairs classrooms.

Unknown vegetation was growing in many locations throughout the fenced in play area that was off site. The outdoor play area must be free from hazards.
In a review of three children's records, all three were missing current documentation of off site consent. One was missing a transportation plan. One did not have documentation of a current progress report.
Documentation of a fire drill for October 2023 was missing from the records during the visit.
Within the Preschool Classroom, an Art Cabinet was unlocked and had Saline Solution accessible to students. The warning label outlined to keep the solution out of reach of students. The office door was open and the preschool students had access to toxic materials such as potting soil, a can of paint and liquid white out.
In a review of three children's files, a Medication Consent From was missing the dates the medication should be provided, proper reasoning for the medication, possible side effects of the medication, directions for storage, and indication of where to find the Health Care Practitioner's signature. There was a medication administration log, documenting three different students receiving over the counter medication. Per EEC regulations, each student requires written consent for the use of unanticipated, non-prescription topical and non-prescription medications. Each child requires a record of any medications administered to the child.
Upon entering the Preschool Classroom upstairs, the gate to the classroom was left open at the top of the stairs.

A review of all staff records indicated the following: -Five did not have evidence of a completed educator orientation. -Four did not have evidence of a current physical examination. -One did not have evidence of meeting teacher qualification requirements. -Two did not have evidence of bi-monthly observations since they were hired. -One annual evaluation was not signed by the Educator.
A review of the Health Care Consultant agreement indicated that the Consultant is not a licensed physician, nurse practitioner or physician’s assistant with pediatric or family health training and/or experience.
It was observed that the changing table on the first floor was not located next to or adjacent to running water.
