At the time of the visit, in a review of the staff meeting log, the documentation was not fully complete. The program was missing documentation of the November 2025 staff meeting. In programs with four or more staff the licensee must develop and follow a written plan and must document regular staff meetings of at least two hours per month to consult with educators on program issues; program planning; policies and procedures; parent communication; implementation of the regulations; children's behaviors and meeting the individual needs of children.
At the time of the visit, within the Infant/Toddler One classroom there was a play grill that was unsecure and tippable. The licensee must only use indoor and outdoor equipment, materials, furnishings, toys, and games that are appropriate to the ages, needs and developmental level of the children enrolled. They must be sturdy, safely constructed and installed and non-tippable.
Individual Health Care Plan (IHCP) - R Apr 2025
At the time of the visit, it was observed that a student within the infant classroom was self feeding a bottle while laying on a boppy pillow. At the time of the observation, an educator was sitting next to the student, working with other students who were not having a bottle. EEC informed the educator that infants must be held while they are being fed a bottle. The educator picked up the student to continue the bottle feeding.

The following were observed indoors: -One dusty overhead vent. -One vent that was rusted or had chipped paint.
On the playground, there was chipping paint around the railroad ties and on the shed. In addition, there was rotted wood on the shed and two exposed screws/ nails. There were exposed and protruding nails/ screws on the side of the building.
This investigation inspection recorded no violations or advisories.
It was observed that there was a plastic sandwich bag, part of a paper plate and a broken plastic cup around the play structure.

At the time of the visit, it was observed that the play kitchen within the Infant/Toddler Two classroom was not secured, was tippable, and had two screws protruding from the back (from where it was once secured). It was observed that the tool bench within the PS/SA classroom was not secured and was tippable. The licensee must ensure that the physical facilities are safe, clean, in good repair and free from hazards and clutter. The licensee must monitor the environment daily to identify and remove or repair any hazards that may cause injury to children. The materials and furnishings used indoors and outdoors must be sturdy, safely constructed and installed, non-tippable, flame retardant, easily cleaned, and free from lead paint, protruding nails, rust, and other hazards that may be dangerous to children.
At the time of the visit, in a review of the emergency evacuation log, there was not documentation of the number of children present for the January 2024 drill. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each monthly drill.
At the time of the visit, in a review of three IHCP's all three were missing documentation of educators trained on the health condition and consequences if medication was not provided. One student had an expired IHCP. The licensee must maintain as part of a child's record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner.
At the time of the visit, in a review of three IHCP's and their corresponding medications, one students emergency medication was expired. Another students medication was not in the original packaging with the prescription label affixed. All prescription medications must be in the containers in which they were originally dispensed and with their original labels affixed. Medications on site cannot be expired.
At the time of the visit, in a review of three IHCP's and the medication consent documentation related to the conditions, one student had an expired medication consent. Another student had a medication consent form for a medication that was not on site, or part of their IHCP. This student was missing the medication consent documentation for their prescribed (REDACTED) that was on site.
It was observed that bookshelves through out the program were tippable; In the Toddler 2 classroom, the blue and white plastic shelf was tippable; In the PreK classroom, there were brooms and shovels accessible to children in the unlocked closet.
It was observed that in the PreK classroom, the ceiling vent near the entrance was dirty and the tile cover was not fitting properly; In the Preschool boy's bathroom, the ceiling vent was dirty and there was an uncovered drain stopper protruding between the faucet handles.

It was observed that a staff member in the Preschool classroom spoke to a child in an inappropriate and harsh tone and was heard telling the child to "shhh" and "sit over there".
It was observed that a staff member in the Preschool classroom did not follow the program's child guidance policy that was posted in the classroom. The staff member did not model appropriate behavior when speaking to a child.
It was observed that in the Preschool classroom, the garbage bin next to the sink was uncovered.
It was observed that there was no staff schedule posted in an area easily visible to educators, parents and visitors.
It was observed that in the Toddler 2 classroom, [REDACTED] were stored in a child's backpack; accessible to children.
The following were observed: in the Toddler 2 classroom, there were food storage containers along the side of the diapering sink; in the Preschool classroom, there were paint bottles, a toy car , a child's sippy cup and a travel mug on the handwashing sink.
Emergency medications were all stored in the kitchen, not readily available to staff in each classroom.
It was observed that in the kitchen, the thermometer in the refrigerator read twenty-six degrees
It was observed that in the Preschool classroom, there were plastic gloves and plastic bags accessible to children in the unlocked cabinet under the sink; In the Infant classroom, there were plastic bags accessible to children in the unlocked cabinet under the changing table.
The building inspection certificate expired on 10/31/19, the fire inspection was expired on 9/19/19
