Four child records were reviewed and found they were all missing one progress report. All preschool age children must have a progress report conducted every six months.
During the visit, two water bottles were not labeled with children's names. Personal items intended for individual use by children, including but not limited to bottles, pacifiers, toothbrushes and sleeping materials, must be labeled with the name of the child for whom they are intended.

During the visit, five electrical outlets in the Fellowship Hall within reach of the children were not covered. All electrical outlets within the reach of children younger than school age must be made inaccessible by use of a safety device or covering that prevents access to the receptacle openings. If the covering is a shock stop, it must be of adequate size to prevent a choking hazard.
A copy of the EEC license was not posting at the time of this visit.

Three children's records were reviewed during this visit. One child's transportation plan was incomplete. The parent/guardian did not indicate how the child would arrive and leave the program.
Four staff records were reviewed and found one staff information form was incomplete (missing the signature and date of the staff).
Children were observed to eat snack prior to washing hands. Hands must be washed immediately before eating. The educator stated the children did wash their hands but the children were observed in the classroom touching classroom materials and then transitioned to the Assembly Room to eat snack without washing their hands in between.
Previously cited during a visit on 1/9/2025: Three child records were reviewed and it was found all three records were missing their date of admission, developmental histories, and the enrollment forms and CPR/First Aid authorizations were not dated. One child record was missing a medication consent form signed by the parent. One child record was missing evidence of a lead test. Corrective Action Plan Submitted: 1. During the parent round-table meeting end-of January, all children's records will be reviewed and any missing information will be updated on the spot, including developmental histories, any missing authorizations, and any medical/enrollment info. Signed forms will be included in all the children's binders. Today's visit: Three child records were reviewed and found to be incomplete. The Child Enrollment Forms and First Aid/CPR consent forms were not dated and the developmental histories were missing.
Emergency medications readily available
Previously cited during a visit on 1/9/2025: "The list of children's medical needs was not posted. In a manner that protects the privacy of each child: 1. a list of all emergency or life saving medications, including but not limited to epinephrine auto-injectors, inhalers, and anti-seizure medications, that specifies to which children they belong; and 2. a list of allergies and/or other emergency medical information provided by the parent for each child. Corrective Action Plan submitted: Information of the only child with an IHCP is now included in a concealed envelope in every classroom. All information pertinent to medications, inhalers, allergies, etc., are included in a manner consistent with privacy protection. Today's visit: A list of the children's allergies, medical info, special diets, and emergency medications were not posted in any areas of the program.

The classroom emergency bag was sitting on a low counter in reach of children. This emergency bag contained one child's emergency medication. Educators must store all medications out of the reach of children and under proper conditions for sanitation, preservation, security and safety during the time the children are in care.

Previously cited during a visit on 1/9/2025: The classroom emergency bag was sitting on a low counter in reach of children. This emergency bag contained one child's emergency medication. Educators must store all medications out of the reach of children and under proper conditions for sanitation, preservation, security and safety during the time the children are in care. Corrective Action Plan Submitted: The emergency bag has been moved to an even higher location that is not accessible to the children at any time. Today's visit: Once the emergency bag containing emergency medication was retrieved and brought into the Assembly Room where the group was located, the bag was placed on the floor near the door within reach of children.
A consent form allowing staff to administer a child's emergency medication was not signed and dated by the child's parent/guardian.
An IHCP is missing for a child with (REDACTED).
During the visit the group transitioned into the Assembly Room and the backpack with emergency medication was left in the hallway outside of the classroom.
The program provides snacks for the children but the administrator was unable to provide a menu by the time this report was published.
An area approximately three feet from the ground next to the Exit Door to the left of the kitchen has peeling paint and exposed plaster.

The following was observed to be accessible to the children: A bucket of uncovered ice melt was sitting by the side entrance/exit that the program would use during an emergency evacuation and five ceiling tiles in the Gross Motor Room were stained.

The evacuation drill was observed and found a drill was not conducted in October 2024.
