One child's two medications were not in their original packaging. All prescription medications must be in the containers in which they were originally dispensed and with their original labels affixed. Over-the-counter medications must be in the original manufacturer's packaging.

Two staff records were reviewed and found one record is missing evidence of a current physical.
The following was observed to be accessible to children: in the upstairs bathroom, the floor tile is cracked and has brown stains in many places; in the coat room area on the main level, the small couch has two rips where the couch stuffing is visible; in PERIWINKLE, the circle run edges are curling up on the far side creating a tripping hazard; in GREEN classroom and the upstairs bathroom, the windows with the screens on the inside of the glass have a layer of dust between the screen and glass; and in the YELLOW classroom downstairs, the outlet to the right of the built in cabinet and drawers is loose, the bookshelf next to this outlet has a lot of visible sticky residue that cannot be disinfected and the built in cabinet and drawers have chipping paint.
Two children's individual health care plans were missing and one was not complete (missing names of educators trained to administer medication).
One child's record is missing medication consent forms for two medications.
Personal Items Labeled
Five children's bottles in the YELLOW ROOM were not labeled with the children's names. Personal items intended for individual use by children, including but not limited to bottles, pacifiers, toothbrushes and sleeping materials, must be labeled with the name of the child for whom they are intended.
The following was observed to be accessible to children: In the GREEN ROOM, contact paper on a low shelf was peeling off into small pieces that children could put in their mouths; and in YELLOW ROOM, one educator's purse was sitting on the first windowsill with the strap hanging down within reach of children, tape residue was visible on the shelving units under the two windows, and a piece of wood paneling on the wall under the first set of windows was loose and peeling back.

The following was observed to be accessible to children: -In the GREEN room, the window crank on the right side has a broken plastic base and sharp pieces. -In the foyer entrance, a child-sized fabric loveseat cover is ripped and the filling is exposed.
The First Aid Kit was missing a CPR mouth guard.
The following was observed to be accessible to children: -The door to the YELLOW BATHROOM is chipping and pieces are lifting on the front at the bottom. -In the PERIWINKLE BATHROOM, a toilet brush was found in the unlocked cabinet under the sink. -In the YELLOW ROOM, a screw is missing on the play wooden stove from the front left corner and could pinch children's fingers if they move the pieces of wood.

The following was found to be accessible to children: -The free standing basketball hoop and the plastic kitchen set were tippable and not secure.
