Exit Safety
During the visit, it was observed that the educator has not logged attendance since March 6, 2026. It was also noted that there were several additional weeks where attendance was not documented. The educator stated that she had two school-aged children in care during the December 2025 school vacation week; however, no attendance records were available for those children.
Current progress reports were not available for three of the enrolled children. One of the children was newly enrolled and did not require a progress report yet.
During the visit, it was observed that the educator has not completed the fire drill for February 2026.

During the visit it was observed educator's CPR/FA expired on 2/2026 and has not been renewed.
The records for the four enrolled children were reviewed. Three children were missing annual updates, and three children were missing proof of current medical documentation, including physicals. Three enrollment packets were expired, and one child’s record was not available for review. One child also did not have a completed emergency card. The educator has been cited for similar issues in the past.
During the visit, several safety hazards were observed throughout the home. In the bathroom, items included, but were not limited to, razors, blow dryers, and clippers, which were accessible to children. A children’s potty-training cubby contained urine and had not been cleaned. In the kitchen, cabinets were not locked and were accessible to children. Cords longer than six inches were within reach, and under the sink, multiple cleaning products were observed, including, but not limited to, bleach, cleaning powder, and detergents. Kitchen drawers contained items such as screwdrivers and nail files that could pose a risk to children. In the living room, an earring was observed on a table, presenting a choking hazard. Additionally, multiple outlets throughout the home were not covered and were accessible to children.

During the visit it was observed the educator’s license was not posted.
During the visit, it was observed that a (REDACTED)child in the educator’s care did not have appropriate napping materials. When asked where the child naps, the educator indicated an infant rocking seat.
During the visit, multiple pillows were observed on top of the changing table.
During the visit, it was observed that the educator currently has a child with documented allergies, but the information was not posted.
educator stated there's two cars that she sometime uses to transport the kids . Upon reviewing it was observed one of the car registration has expired since 11/2025
One exit did not have an exit sign. In addition, multiple cushions were observed blocking one exit, posing a safety hazard. It was also observed that an exit sign was posted on a door that was not an actual exit.
During the visit, the fridge temperature was observed at 42°F. It was discussed that the temperature must be maintained between 32°F and 40°F.
This follow-up visit inspection recorded no violations or advisories.
The records were reviewed for the one enrolled child. The Annual Update form was missing and the child needed proof of current physical, immunizations, and lead screening.

The carbon monoxide detector was working in the basement, however there was no other carbon monoxide detectors anywhere else in the home. Please send proof of carbon monoxide detectors installed on each floor level of the home.

There was no proof of attendance records for August, September, and October.
The educator and three household members currently do not have approved background record checks.
The educator reports she has not been completing the evacuation drills.
The first aid kit was missing the child’s emergency card, tweezers, a thermometer, gloves, medical tape, and bandaids (there was one bandaid in the kit.). The educator also reports she does not have a first aid kit in her vehicle and also does not have a seat belt cutter in her car.
There was multiple hazards accessible in the bathroom drawers and under the sink cabinet (cleaning products, personal care products, shaving razors, cords to hair appliances, etc.)
There was a working smoke detector in the basement, but there were no other smoke detectors in the home. Discussed smoke detectors must be installed on every floor level. Please send proof of installed smoke detectors.
The educator reports she has not completed any professional development training for this year, including the EEC Essentials 2.0 trainings. She currently has 21 hours of professional development training. Please send proof of at least 9 hours (could be included with the 12 hours from the EEC Essentials 2.0)
The educator is using disinfecting wipes to clean the table. Discussed this is too strong and a sanitizing solution needs to be used instead so children do not ingest the chemicals.
The records were reviewed for the one enrolled child. The record was missing the Annual Update form.
