The following was observed to be accessible to children during the visit: In ROOM 12, the 4-cubby shelf and the lateral bookshelf were not secure and tippable.
Three staff records were reviewed and found one staff record did not have a current PQ registration (expired 3/22/2025).

One child's (REDACTED) was not available during the visit and a second child's antihistamine had expired in 9/2025.

The use zone around the slides and climbers had approximately two inches in depth of mulch. These areas need to have at least nine inches of mulch.

The First Aid Kit in ROOM 14 did not have scissors or ice packs.

The following was observed to be accessible to children: -In ROOM 10, three pairs of adult scissors were in cups on a low bookshelf. -In ROOM 12, the four windows had long ribbons attached to the shade pulls.
One child's Albuterol was in an old Albuterol box (expiration date on the box and on the inhaler did not match) and was missing the label from the pharmacy.

Medication consents were reviewed and found: -One child's medication consent had expired (REDACTED).
The Building and Fire Inspection is expired (11/1/2023).
The following was found to be accessible to children: -In CLASSROOM 10, the small area rug in the doorway of the classroom was lifting at one corner and is a tripping hazard. -In CLASSROOM 12, multiple shelving units lined up along the wall under the windows were not secure and tippable.
A review of the emergency medications was conducted and found: -Antihistamine for two children was not available.

A written parental consent for staff to administer an emergency medication was missing from one child's record.
The following was observed to be accessible to children: -In ROOM 10, two strings of holiday lights hung loosely along the bottom of two bulletin boards, the telephone cord hung loosely from the outlet, an electric outlet did not have a safety cover, and broom was leaning against the cabinet. -In ROOM 12, one electric outlet below the chalkboard did not have a safety cover, and a box of gloves sat on the ledge along the back wall (both items were fixed during the visit). -In ROOM 14, a box of sandwich bags was found in the drawer labeled "Kitchen Supplies", the phone cord hung loosely from the outlet, and the play kitchen set was not secured to prevent tipping. -In ROOM 16, the children's bench cushion has ripped corners and the plastic table clothes used to cover shelves of toys and supplies during rest time is shredding and could become a choking hazard to sleeping children laying close by. -In the bathroom at the end of the hallway, a roll of bags was found in the cubby system (the bags were moved during this visit).
A review of five children's records indicated the following: -Three did not have documentation of who trained the educators to administer the medication to the child. -One did not have documentation of which educators were trained to administer the medication.
An injury that occurred on (REDACTED) did not have documentation of the time of the injury as required.

Individual Health Care Plans were reviewed for children with chronic medical conditions prescribed medication and found: -One child's IHCP was not complete- missing medical treatment. -One child's IHCP was not complete- missing who trained staff to administer medication. -One child's IHCP was not complete- missing consequences of not receiving treatment. -One child's IHCP does not include that antihistamine is part of the plan.
All of the emergency medication was reviewed and found: -One child's (REDACTED).
During the visit, the Educator in CLASSROOM 10 walked the children to the bathroom down the hall and did not take the attendance with them. As stated in regulation 7.04(4)(a)2, Educators must always have a method of knowing exactly who is present on the premises at any given point in the day.
The following was observed to be accessible to children in the outdoor space: - The play kitchen was not secured to prevent tipping -There was not an adequate amount of impact absorbing materials under and around the fall zones. The mulch was approximately two inches in depth.
