During the previous visit on 5/8/2025 (#159693) this technical assistance was provided: Ensure educators are in same space as their assigned children at all times even when outdoors. Educators must be in sufficient proximity to children at all times in order to be able to intervene quickly when necessary. During this visit, the following was observed: One preschool educator brought a child into the building to use the bathroom. Although the educator stood in the doorway of the building to supervise the child inside and try to maintain ratio outside, the educator left three educators alone with two preschool groups for a total of 36 children (PreK had 17, Dolphins had 20, minus the child in the bathroom). During the visit, it was also observed that four different groupings spent outdoor time together. The Infant/Toddler group (toddlers only now) was combined with the Toddler/Preschool group and the Preschool was combined with the Preschool/School Age group. Outside of the first two and last two hours of the day, different groupings cannot be combined.

Emergency medications readily available
The Building Inspection is current and expires 5/1/2026. The Fire Inspection is not current (expired 4/27/2026).
During the visit, the following was observed: In the Lions classroom, the green baseboard along the bottom of the walls had a lot of chipping paint.
During the visit, one child’s inhaler was not current (expired 12/2025).
During the visit, one child’s emergency (REDACTED) was not available at the program.
Weekly snack/meal menu made available; Previous menus on file for one year
Three staff records were reviewed and it was found one staff record was missing evidence of completion of EEC Essentials 2.0 (hired 3/20/2025).
The following was found to be accessible to children: A red rug set within the sand on the right front part of the yard was uncovered and curling back (tripping hazard); a free standing basketball hoop was found in the far yard; and a couple of the yellow and black metal Tonka trucks were observed to have chipping paint and rust.

In OWLS 2, the attendance was not accurate. Ten children were marked as present but only nine were in the group when licensor observed the classroom.
The Building/Fire Inspection is not current. It expired 4/1/2024.

The following was found to be accessible to children: -In the Preschool Classroom (second door on right side), paint is peeling to the bottom left of the American Sign Language Poster on the internal wall.

Medication
The attendance in the Preschool room (first door on the right) was incorrect. Nine children were in attendance but only six children were checked in. None of the attendance sheets included the times of arrival and past dates did not have times of departure recorded. Daily attendance records must indicate each child’s attendance, including arrival and departure times.

A review of medication consent and Individual Health Care Plans indicated the following: -One did not have a medication consent for medication at the program. -One did not have a written Individual Health Care Plan for prescribed medication at the program.
The program did not have a current Building and Fire Inspection from the local town.
A review of educators files indicated the following: -One did not have current First Aid Certification as required.
A review of five children's records indicated the following: -One did not have a current Individual Health Care Plan.

Medication Administration Policy
A review of five Individual Health Care Plans indicated the following: -Five did not have documentation of who trained educators to administer the medication and which educators were trained to administer the medication.

The following were observed: -Lions: A staff office was accessible. Inside a Body Fluid Spill Kit was accessible. In addition, two shelves and a play kitchen were tippable. The connector piece to the toilet was rusty and accessible to childen. -Dolphins: there was a sharp knife accessible to children. -Duckling: The trash can was soiled and the dish drainer was rusty. -Monkey: A draping cord was over 6 inches and accessible to children. -A step stool in the bathroom was soiled.
The program did not have a budget available. The licensee must have and submit a projected one-year operating budget that estimates income and expenses.

In OWLS 2, four children's water bottles were not labeled with their names. Personal items intended for individual use by children, including but not limited to bottles, pacifiers, toothbrushes and sleeping materials, must be labeled with the name of the child for whom they are intended.
The program did not have a current or past menu. Full detailed listing specifying breakfast, lunch, and snack; Previous menus should be maintained in writing for one year.
The following was observed to be accessible to children: In OWLS, the outside blue wall had many small areas of chipping paint.
A list of children's chronic medical conditions/needs were posted in each classroom in a manner to protect their privacy but the medications were not listed. In a manner that protects the privacy of each child: 1. a list of all emergency or life saving medications, including but not limited to epinephrine auto-injectors, inhalers, and anti-seizure medications, that specifies to which children they belong; and 2. a list of allergies and/or other emergency medical information provided by the parent for each child.
All emergency medications for children were reviewed and found: -One child's antihistamine was not available to staff as the parents had not provided it. -One child's antihistamine was expired (expiration date- 3/2024).
Two evacauation drills over the last two years did not indicate the specific date of the drill.

A review of three children's records indicated that the enrollment sheet in one file was not updated annually.

The program license was approved to care for children up to age 8 years old. On the day of the visit, the program was providing care for one child(REDACTED).
A review of staff meeting documentation over the last 2 years indicated that the length of the meeting was not recorded 16 times.

The following were observed outdoors: -Two ride on toys were cracked with pinch points. -A piece of the fence was broken. -Two rugs were curling and partially concealed by sand. -A child safey gate was broken. -Two shovels were leaning against the fence and accessible to children.
In the I/T room, a list was posted that identified children and medical conditions. The list did not include information about the medication used to treat the condition and was not privacy protected.
A spray bottle in the Dolphin room was not labeled and was accessible to children on the counter.
There was chipping paint in the Duckling room. There was chipped plaster in the entry way on the ceiling.
