The Educator did not have any staff records for review.
A review of 12 children’s records show that: 2 children just have immunizations on file, no medical. 2 children need proof of a lead test, 2 general permissions were not signed and 1 child need the parent handbook,and parental visit notification signed

The second exit was blocked by nap mats today. The educator moved them while the licensor was present.
The licensor observed the following: in the basement there were wires accessible, there was a mop accessible to the children in the bathroom

The Educator has completed her 30 hours of training but has not completed 10 hours in diverse learners.
When the licensor arrived a (REDACTED) was in the pack and play asleep on her stomach. The mattress was not installed properly, leaving a gap on both ends and the sheet was not tight fitted. This is the educator’s 2nd safe sleep non compliance.

The educator’s attendance records did not document the days/times the assistant was present, which does not allow an accurate method of knowing who is on the premises. Additionally there are multiple days a week where there are more than 6 children present, including more than 3 infants and it is not clear if an assistant was present with the educator.

There was no gate at the bottom of the stairs today on the first floor
The first aid kit was missing the emergency card information and the instant cold pack. The educator added the cold pack while the licensor was present
The licensor observed a few hazards in the yard: there are several snow shovels accessible, an old metal table umbrella stand, a pile of cinder blocks holding an umbrella that are unsafe and a tipping hazard, and a pile of bricks behind a tree stump. Please send pictures of the completed non compliances

The outdoor space had several hazards including but not limited to: wood pile and cement blocks were accessible, the gate around the inground pool had fallen down in one spot. The educator attempted to fix it with a baby gate and green plastic fencing, neither of which was sturdy or measured 4ft. The inground pool was filled with cement blocks and other hazards. Due to this the backyard is being removed from the licensed space at this time.

When the licensor arrived she observed a 7 month old asleep in the car seat. He was strapped in, wearing a bid and had a blanket on him. The certified assistant removed the infant from the car seat. The educator stated the child had just arrived, however the attendance records show he arrived 35 minutes prior to the licensor’s arrival
The licensor observed a preschool age child in a toddler high chair during circle time. The Educator stated it was because the child did not sit for circle time.
The licensor observed a koi pond being built in the backyard. The koi pond is accessible to the children in the backyard space. The wall measures less than 4ft. At this time the backyard cannot be used.
The licensor observed that the cabinet under the bathroom sink was not locked and 3 bottles of cleaning supplies were accessible. In the kitchen the cabinet under the sink was not locked and 2 bottles of cleaning supplies were accessible. There was laundry detergent accessible to the children on the floor of the downstairs bathroom. In the basement there were wires and an oxygen tank accessible to the children

A review of the records show that: 2 children need current physical/immunizations, 1 child was missing the required information on how to contact parent during child care hours, 1 was missing the signature and date for acknowledgment of parent handbook, 1 was missing an annual update, 1 general permission was incomplete. 2 children have documented allergies and on Individual Health Care Plan.
The educator does not have training hours in the assistants files
The educator did not have any progress reports for review today. She stated that she completed the progress reports every 6 months and then sends them home with the parents
The first aid kit did not contain the correct emergency contact cards for them children in care, the instant cold pack had popped making it unusable, misisng guaze roll
The temperature in the fridge was 46 degrees, which is above the 32-40 degrees in the regulations
There was a window in the kitchen that was open from the bottom today. There was no window guard in place preventing it form opening more than 2 inches.
There was no gate on the stariway leading to the second floor from the living room.
There is a plastic bag hanging on the gate at the bottom of the basement stairs accessible to the children. The seasonal porch has hazards including but not limited to: vacumn, glass bottles accessible on a low shelf, and it was very cluttered and no room to move around the space. In the kitchen there were 2 drawers that the lock was broken and contained hazards such as scissors, sharp knives. In the bathroom the cabinet under the sink the lock was broken and hazards such as Clorox wipes and other cleaning supplies were accessible.
The second exit from the first floor was not free hazards.
The inground pool does not have secure fencing around it preventing it from being accessed by the children. Additionally the pool contains hazards such as: cement blocks and branches
