Children’s records were reviewed. There were three files that had missing lead screening results, Two files had incomplete physicals that did not have immunization records. There were two files that had missing sections within the enrollment packets; this was reviewed with the educator.
Two files did not have a current progress report
It was observed that the educator did not wash her hands or child’s hands after diapering. It was observed that educator washed a child’s hands after diapering in the kitchenette sink. Handwashing after diapering should only be done in the bathroom sink.
Parent handbook does not have EEC’s contact information and educator has not added assistant information.

Educator has six children enrolled of which four are under 2 years; three are infants. Educator has an assistant. The assistant was present during today’s visit, however the educator is not documenting daily attendance for her assistant compliance for ratios could not be verified.
Educator is not keeping accurate daily attendance for all children enrolled.
At time of visit, when licensor entered the space, it was rest time. One child was sleeping and the the other two children were not yet asleep. It was observed that the room appeared to be extremely dark. The educator at time of visit did open one of the curtains to a window, which gave the room sufficient lighting. It was discussed with the educator that rooms in which children nap cannot be completely dark in order to be able to appropriately supervise.
