Children’s files reviewed were all due for updated progress reports. Based on ages of children enrolled educators should be documenting a progress report every 6 months.
Educator’s PQ number expired on 4/2025 and must be renewed.

A random review of children’s files was completed. It was observed that from the files reviewed eachl child’s files needs to be reviewed for annual update on enrollment packets and updated physicals.

It was observed that small slide was set on hard top and not on grass area.

Pablo did not have an updated physical on file and did not have proof of ongoing professional development trainings for 2022-2023 Dayanara Tejeda regular assistant expires 7/2025. Kiara Tejeda regular assistant expires 6/2025 there was no updated physical on file.

Attendance records were reviewed. Attendance records for 5/19/2022 reflected four children not being signed out and educator is not documenting assistant’s daily attendance

Children’s records were reviewed. One file did not have an updated physical, one file did not have lead screening, and another file did not have the physical description completed on the first page of enrollment packet.
Attendance records were reviewed. Attendance reflected in and out times for children. Today there were 4 children present only 3 signed in arrival time, fourth child only had parent initials for arrival time. Discussed with the educator having her assistants sign in and out when they are present and functioning as her assistant.
Parent handbook did not include assistants that currently work with the educator.
First aid kit was missing one child emergency card and two emergency cards had no signature date.
Bathroom cabinet had access to cleaning products, child proof lock was not latched. Bathroom lower shelf has access to beauty products and cleaning products. There’s access to workout bicycle
Smoke detector in lower level living room did not go off.
Carbon monoxide detector in lower level living room did not go off.
There is no gate to prevent access to stairway leading to first floor.
There was a small slide under 18 inches that was placed on the brick surface instead of the grassy area.
