At the time of the visit, three children with IHCP's were reviewed and the following was found: one child's IHCP and medical consent forms did not include all EEC required information, and one child's medication was not accompanied by IHCP.

At the time of the visit three children’s files were reviewed and the following was found: one file had expired off site consent, transportation plan, and developmental history forms, and two files did not have current physicals.
