The licensor observed the Yellow Toddler and Purple School Age programs do not have evidence of a plan of program activities; the Green PreK classroom has a calendar plan of activities posted for 1/2026.
The staff schedules posted in the program are not current. Staff Members hired under EEC Minimum Hiring Requirements have exceeded the 6-month extension period with no evidence of completing the requirements for EEC certification.
The licensor reviewed 3 children's files with the following findings: 2/3 children do not have a current progress report on file; 2/3 children have an outdated annual physical on file.

At the time of the visit three staff files were reviewed and the following was found: two files were missing staff information forms, one files did not have evidence of MMRs, one file did not have a current physical, and one file did not have first aid and CPR certification

Licensor observed the following: cabinet under the toddler classroom sink contains a plunger and refill container of handsoap that are accessible to the children; the blue tiled bathroom has a plunger next to the toilet and a toilet brush in the cabinet under the sink that are accessible to children; the first aid kit in the preschool classroom was on the floor behind the teacher's desk and is accessible to the children.

The following was observed for 4 infants in cribs. 1. One sleeping infant was observed with a blanket in their crib. 2. One infant was in a crib with a wedge positioner and blanket. 3. One sleeping infant had a blanket, a toy, and a hanging object in their crib. 4. One infant was observed with an I pad hanging on the inside of their crib. 5.Two infant cribs did not have a fitted sheets.
Upon the first visit in the infant classroom, one infant was observed drinking a bottle while in a bouncy seat. Two infants were observed laying down in a their cribs drinking their bottles. EEC reviewed the regulations, indicating that infants need to be held when drinking a bottle, and the educator removed the bottle from the child. Upon the second visit into the infant classroom the same child was drinking a bottle while in the same bouncy seat.
Chipping paint was observed in the Infant room.
