Children's Records
A random review of children’s files was completed. It was observed that multiple files are due for the annual update for enrollment packets. It was also observed that some files had the general permission signed but did not indicate where children are taken when off premises. It was observed that there were missing parent signatures and signature dates for some of the enrollment pages and some pages did not have educator’s name as required. Educator must review all files.

Children’s records were reviewed. It was observed that all files need to have the general permission completed to indicate where children are taken when off premises. One file had a missing physical and lead screening. Various files had missing signatures, signature dates, missing physical description on first page of enrollment packets, missing face-sheet. This information was emailed to the educator.
CPR/First Aid - R Apr 2025
Educator’s first aid expires on 10/2024 and CPR expired on 10/11/2023. Educator must submit proof of renewed CPR by 11/6/2023.
This follow-up visit inspection recorded no violations or advisories.
Space (health risks) - R Apr 2025
Safe Sleep
It was observed that in one of the rooms on the basement level there was access to a Raid can and a portable fan. Door to room was closed therefore children did not have access, however room is part of licensed space.
It was observed that in the pack and play where (REDACTED) infant naps had a padded mat on top of pack and play mat and had a blanket over both mats. Infant was not napping at time of visit. Safe sleep regulations and policy were reviewed with the educator. Safe sleep parent letter and safe sleep pledge was given to the educator. The safe sleep policy was emailed to the educator. The educator will be required to retake safe sleep training and follow up visit will be conducted.
